Dear Mr. Apariz & Ms. Hernandez:
Forsyth County tax records indicate that you are the current owners of the subject
property. Please be aware that this property has a permit to discharge treated domestic
wastewater to Lowery Mill Creek, which is classified as Water Supply III (WS-III) waters
and is located in the Yadkin-Pee Dee River Basin. The general permit to discharge is
NCG550000 and the certificate of coverage issued for this specific property under
NCG550000 is NCG550057.
Ron Boone, of the Winston-Salem Regional Office (WSRO) of the NC Division of Water
Resources (DWR or the Division), attempted to meet with you on September 19, 2018,
the property to conduct a compliance evaluation inspection (CEI) of the wastewater
treatment system, however, you were not there and neither was the home. After further
investigation, it was revealed that the home burnt down in July 2012. It appears that you
then purchased the property in March 2015.
Please contact Mr. Boone by phone at your earliest convenience at 336-776-9690, or by
email at ron.boonencdenr.gov to discuss whether you should keep the permit or
request rescission. Keeping the permit will ease the process of reestablishing sewer
service at the property in the future if you ever rebuild a new home there, but you would
be required to pay annual permit fees. Rescinding the permit would inactivate the current
permit and you would not have to pay annual fees, but if you ever rebuild a home on the
property you would have to apply for and be issued an all new discharge permit. Please
note that fees for this permit, which are currently $60 per year, have not been paid for the
past 7 years.
