1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
The design options Were limited because the waterline had to utilize a portion of the existing utility corridor that parallels Green Level West Road. All
stream crossing are perpendicular and Wetland crossings have been minimized as much as is practicable while still meeting the project's purpose and
need.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
The engineer has designed the project to minimize impacts as much as practicable. Apex requires a 15-11
maintenance corridor for a waterline such as
this one. The stream and Wetland crossings will be returned to pre -construction contours and revegetated with native vegetation.
2. Compensatory
Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
C' Yes 6 No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:
the temporary wetland and stream impacts are under the mitigaiton thresholds specified by the Section 404 NWP program and the Section 401 regulations
