ROY COOPER NORTH CAROLINA Governor Environmental Quality MICHAEL S.REGAN Secretary RECEIVEDIDENRIDWR LINDA CULPEPPER Interim Director AUG 0 7 2018 July 30, 2018 Water Resources Permitting Section Ms.Stacy Burns Betsy Jeff Penn 4H Center 804 Cedar Lane Reidsville, NC 27230 Subject: Compliance Evaluation Inspection Report NPDES Permit#NC0078271 Betsy Jeff Penn 4H Center WWTP, Rockingham County Dear Ms. Hopper: On July 16, 2018, Paul DiMatteo of the Division of Water Resources(Division),Winston-Salem Regional Office, conducted a Compliance Evaluation Inspection of the above referenced wastewater treatment system.Jeff Johnson Operator in Responsible Charge (ORC), and William Amos, backup-ORC,were present for the inspection.This type of inspection consists of two basic parts: an on-site inspection of the facility and a review of facility files and self- monitoring data. Observations from each area of the inspection are summarized below. Site Review 1. Permit The current permit became effective on June 1, 2017 and expires on April 30, 2022. Permitted septic tanks, effluent filters, an E-Z Treat System with three pod zones, and ultraviolet disinfection. 2. Facility Site Review The facility was as described in the permit.The facility is permitted to discharge a monthly average of 13,100 gallons per day(gpd);typical daily discharge is approximately 2,450 gpd.An automatically activated standup generator is available that Mr.Johnson said can power the entire plant. 3. Operation and Maintenance The plant is operated as required.All operators are properly designated and hold adequate certifications. The facility appeared to be well operated and managed. Flow Measurement Effluent flow is measured using a positive displacement type flow meter. Mr.Johnson checks the calibration yearly by measuring against the 45 degree v-notch weir on the UV disinfection system. 5. Effluent/Receiving Waters The effluent is discharged to Carroll Creek,class C waters in the Roanoke River Basin.A path to the outfall was available and there was no visible impact to the receiving stream. Documentation Review 6. Records/Reports Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMRs include all permit parameters; Copies of DMRs,operator visitation logs,daily operation sheets, maintenance reports, lab reports and Chain-of-Custody forms were available for review during the inspection. 7. Self-Monitoring Program A review of DMRs from August 2015 through June 2018 showed no actioned violations, and a comparison of submitted data with bench sheets revealed no errors or concerns. 8. Laboratory The facility analyzes field parameters at its on-site laboratory under certification#5556. Non-field analysis is performed by Pace Analytical in Eden, certification#633. Copies of all laboratory documentation were readily available. Procedures for analyzing field parameters including required calibrations appeared acceptable. 9. Sludge Handling/Disposal All septic tanks and the cafeteria's grease trap are emptied annually by Billingsly Septic Tank Company. Mr. Johnson said that the invoices are sent to the university system and copies are not retained on site. Please be sure to retain copies of these invoices for facility records.
