CT L.PEPPER Interim Director Subject: Attachment B Certification Permit No. WQ0037135 Southern Soil Builders RLAP Wilkes County In accordance with your permit modification request received April 4, 2018, we are forwarding berewith a modified Attachment B certified May 11, 2018, to Southern Soil Builders, Inc. for the subject residuals program. This certification is being issued to modify Attachment B by: Addition of a total of four fields totaling 38.1 acres, three fields being in Rutherford County totaling 33.4 acres (NC-RU-NMA, NC-RU- NMB, NC-RU-NMC) and one field in Wilkes County totaling 6.7 acres (NC-WK-DK-01). This modified Attachment B shall be effective from the date of certification until June 30, 2019 (i.e., expiration date of the current permit), shall void Attachment B of Permit No. W Q0037135 issued July 3, 2014, that was previously certified on June 6, 2017, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace the Attachment B of Permit No. WQ0037135 that was certified on June 6, 2017 with this modified Attachment B. Please note that only the maps for new land application sites (Figures 10 to 12) are attached to this letter, It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements, and/or limitations contained in this certification or modified Attacbment B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
