This permit shall be effective from the date of issuance until October 31, 2018, and shall be subject to the conditions and limitations as specified therein. An extension shall be requested at least six months prior to the expiration of this permit. Upon receipt of the request, the Commission will review the adequacy of the facility described therein and, if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. 0 Please note Condition 1.3 which requires a past -construction soil evaluation to be performed prior to operation of the irrigation system and pay particular attention to the performance standards and operation and maintenance requirements listed in Section II, III and Attachments A and B of this permit. Note that operation of this reclaimed water system requires analytical monitoring at a frequency of three times per year, which may be reduced to annually after a two-year period based on satisfactory compliance with all permit requirements. Failure to establish an adequate system for collecting and maintaining the required operational information may result in future compliance problems. WQ0036557 Version 1.0 Shell Version 131016 Page 1 of 10 This facility is approved with the following alternative design criteria as approved under ISA NCAC 02U .0105 (ref. 02T .0105(n)): • Dual treatment units for the plate membranes (15A NCAC 02U .0402(b)) were not required. Documentation was provided that replacement of the membranes can be accomplished within the 5 -days of storage time proved by the systems. • The online turbidimeter (ISA NCAC 02U .0402(c)) was not required due to the existence of a pressure transducer that monitors pressure between vacuum pump and membrane and will shut down the system in the event of filter failure. • Back-up power supply (15A NCAC 02U .0402(h)) was not required due to existence of the 5 day upset storage, paired with the likelihood of low volumes of waste being produced in the event of loss of power. • Irrigation rages (15A NCAC 02U .0402(m)). The approved water balance utilized a 30.8% drainage reduction factor. Standard for a 2U non -conjunctive system is 4-10%. The approved annual hydraulic loading rate is 3.7 in/yr above typical values for Single family residence wastewater systems. Approval of the alternative drainage factor, and Ioading rate was supported by the irrigation of reclaimed water, and consulting professional soil scientist. This permit shall be effective from the date of issuance until October 31, 2018 and shall be subject to the following specified conditions and limitations: Y. SCHEDULES In accordance with 15A NCAC 02T .0 116, upon completion of construction and prior to operation of this permitted facility, a certification (attached) shall be submitted from a licensed North Carolina Professional Engineer certifying that the permitted facility has been installed in accordance with this permit, Division approved plans and specifications, and other supporting documentation, including the location of all monitoring wells as applicable. If this project is to be completed in phases and partially certified the Permittee shall retain the responsibili1y to track further construction-gppLoved under the same permit, and shall provide a final certificate of completion once the entire project has been completed. Mail the• Certification to the Division of Water Resources, Water Quality Permitting Section, 1636 Mail Service Center, Raleigh, NC 27699-1636. 2. The Raleigh Regional Office, telephone number (919) 791-4200, shall be notified at least 48 hours in advance (excluding weekends and holidays) of operation of the installed facilities such that an in- place inspection can be made. Notification to the regional supervisor shall be made from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 3. Upon completion of final site grading activities and prior to installation of the reclaimed water iijigation system, a licensed soil scientist shall conduct a post -construction soil evaluation of proposed irrigation areas, and submit a signed and sealed amended soils evaluation report to the Division of Water Resources Central Office and Raleigh Regional Office, The Raleigh Regional Office shall be notified at this time and given the opportunity to evaluate post -construction soil conditions prior to irrigation system installation. In the event the post -construction soil conditions can no longer support the design flows of the wastewater treatment system, the permittee shall recondition the soil or take addition measures to ensure that adequate disposal or benifical use alternatives are identified and incorporated into the permit as necessary to meet the design flow disposal needs of the treatment system. 4. No later than six months prior to the expiration of this permit, the Permittee shall request renewal of this permit on official Division forms. Upon receipt of the request, the Division will review the adequacy of the facilities described therein, and if warranted, will renew the permit for such period of time and under such conditions and limitations as it raay deem appropriate. Please note Rule 15A NCAC 02T .0105(d) requires an updated site map to be submitted with tUe permit renewal application. WQ0036557 Version 1.0 Shell Version 131016 Page 2 of 10 5. If the subject non -discharge facility connects to a public or community sewage treatment system, the Permittee shall request a permit modification to change permit type from non -conjunctive reuse to conjuctive reuse within 180 days of connection. IT. PERFORMANCE STANDARDS The subject reclaimed water facilities shall be effectively maintained and operated at all times so there is no discharge to surface waters, nor any contravention of groundwater or surface water standards. In the event the facilities fail to perform satisfactorily, including the creation of nuisance conditions due to improper operation and maintenance, or failure of the utilization areas to adequately assimilate the reclaimed water, the Permittee shall take immediate corrective actions including Division required actions, such as the construction of additional or replacement reclaimed water generation and utilization facilities. 2. This permit shall not relieve the Permittee of their responsibility for damages to groundwater or surface water resulting from the operation of this facility. 3. Reclaimed water generated from this facility shall not exceed the effluent limitations specified in Attachment A. 4. Application rates shall not exceed those specified in Attachment B. 5. The Operation and Maintenance Agreement (attached) between the Permittee and the Division of Water Resources is incorporated herein by reference and shall be a condition of this permit. Noncompliance with the terms of the Operation and Maintenance Agreement shall subject the Permittee to all sanctions provided by North Carolina General Statutes §143-215.6A to §143-215.6C for violation of or failure to act in accordance with the terms and conditions of this permit. 6. The irrigation system shall be connected to a rain or moisture sensor, which shall indicate when effluent application is not appropriate in accordance with Conditions 11I.5 and 111.6 of this permit. 7. The following shall be requirements for the reclaimed water storage and utilization facilities: a. -All reclaimed water valves, storage facilities and outlets shall be tagged or labeled to warn the public or employees that reclaimed water is not intended for drinking. Where appropriate, such warning shall inform the public or employees to avoid contact with reclaimed water. b. All reclaimed water piping, valves, outlets and other appurtenances shall be color -coded, taped or otherwise marked to identify the source of the water as being reclaimed water. v i. All feclaimed water piping and appurtenances shall be either c lored purple (i.e., Panton 522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER -- DO NOT DRINK" or be installed with a purple (i.e., Pantone 522) identification tape or polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and repeated every three feet or less. ii. Identification tape shall be at least three inches wide and have white or black lettering on purple (i.e., Panton 522) field stating "CAUTION: RECLAIMED WATER — DO NOT DRINK." Identification tape shall be installed on reclaimed water pipelines in a visible manner, fastened at least every 10 feet to each pipe length and run continuously the entire length of the pipe. iii. Existing underground distribution systems retrofitted for the purpose of distributing reclaimed water shall be taped or otherwise identified as noted above. This identification need not extend the entire length of the distribution system, but shall be incorporated within 10 feet of crossing any potable water supply line or sanitary sewer line. WQ0036557 Version 1.0 Shell Version 131016 page 3 of 10 c. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits operation by authorized personnel only. I Hose bibs shall be located in locked, below grade vaults that shall be labeled as being of non - potable quality. As an alternative to the use of locked vaults with standard hose bib services, other locking mechanisms such as hose bibs which can only be operated by a tool may be placed above ground and labeled as non -potable water. The compliance and review boundaries are established at the utilization area boundaries. Any exceedance of standards at the compliance or review boundary shall require action in accordance with 15A NCAC 02L.0106. 9. The Permittee shall apply for a permit modification to establish a new compliance boundary prior to any sale or transfer of property affecting a compliance boundary. 10. In accordance with 15A NCAC 02L .0107(d), no wells, excluding Division approved monitoring wells, shall be constructed within the compliance boundary except as provided for in 15A NCAC 02L .0107(g). 11. The facilities permitted herein shall be constructed according to the following setbacks: a. The setbacks for reclaimed utilization sites shall be as follows (all distances in feet): L Surface waters: 25 ii. Any well with exception to monitoring wells: 100 b. The setbacks for treatment units shall be as follows (all distances in feet): i. Any habitable residence or place of public assembly under separate ownership: 100 ii. Any private or public water supply source: 100 iii. Surface waters: 50 iv. Any well with exception of monitoring wells: 100 v. Any property line: 50 III. OPERATION AND MAINTENANCE RE iTIREMENTS The reclaimed water generation and utilization facilities shall be properly maintained and operated at all times. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any reclaimed water or partially treated effluent resultigg from the operation of this facility. i i 2. Upon the Water Pollution Control System Operators Certification Commission's (WPCSOCC) classification of the subject non -discharge facilities, in accordance with 15A NCAC 08G .0200 the Permittee shall designate and employ a certified operator in responsible charge (ORC) and one or more certified operator(s) as back-up ORC(s). The designated operator shall visit the site at a minimum of once per month. In the event that the visual or audible alarms or telemetry for remote monitoring are found to be inoperable, the visitation schedule shall be in accordance with 15A NCAC 08G .0200, until notified by the Division that the alternative visitation schedule can be resumed. After 12 months of operation the permittee can request that the visitation schedule be further reduced, provided it can be supported that the system can operate at the reduced visitation schedule. The Division does not intend to reduce the ORC visitation schedule to a frequency less than the minimum sampling requirements established in Attachment A of this permit. An operator certified by the Water Pollution Control System Operators Certification Commission (WPCSOCC) of a grade equivalent or greater than the facility classification shall be on call 24 hours per day. 5. The Treatment Tank and the Spray Field Dose Pump Tank shall be equipped with a staff level gauge or equivalent marking system to allow for visual inspection of the water levels in each tank to verify the amount of wetweather storage available, and that that operating conditions are being maintained as to not utilize the 5 -day upset storage. 6. A suitable year round vegetative cover (Fescue or other turfgrass type) shall be maintained on irrigation sites at all times, such that crop health is optimized, allows for even distributions of reclaimed water, and allows inspection of the irrigation system. 7. Adequate measures shall be taken to prevent effluent ponding in or runoff from the irrigation sites. 8. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause ponding or runoff. 9. All reclaimed water irrigation equipment shall be tested and calibrated at least once per permit cycle. Calibration records shall be maintained at the facility for a period of no less than five years, and shall be made available to the Division upon request. 10. Only effluent from Rose Hall Lot 17 SFR shall be utilized in accordance with this permit. 11. No automobiles or machinery shall be allowed on the irrigation sites except during equipment installation or while maintenance is being performed. 12. Public access to the reclaimed water generation facilities shall be prohibited. 13. Public access to reclaimed water utilization sites shall be controlled during active irrigation. Such controls may include the posting of signs notifying irrigation with reclaimed water. 14. Diversion or bypassing of untreated or partially treated wastewater from the treatment facilities is prohibited. 15. All effluent shall be routed to the five day upset tank should the limit for fecal coliform (e.g., daily maximum concentration of 25 colonies per 100 mL) or turbidity (e.g., instantaneous maximum of 10 NTU) be exceeded or pressure failure in the membrane treatment unit, until the problems associated with the treatment facilities have been corrected. Wastewater in the five day upset tank shall be pumped back for re -treatment or treated in the five day upset tank prior to utilization. r 16. The Operation and Maintenance Plan (attached) shall be maintained as part of this permit and signed by all legal homeowners. This document shall be transferred with the permit during any change of ownership and serves as the required education material to inform users (i.e. homeowner) about the proper use of generated reclaimed water and operation and maintenance of the reclaimed water facilities, 17. The residuals generated from the wastewater treatment facilities shall be disposed or utilized in accordance with 15A NCAC 02T .1100. 18. Pump/dosing tanks shall have audible and visual alarms external to any structure. 19. Any excavation into bedrock shall be lined with a 10 millimeter (min) synthetic liner. A water -tight seal on all treatment/storage units or minimum of two feet protection from the 100 -year flood plain elevation shall be provided. 21. Reclaimed water shall not be used for direct reuse as a potable water supply, residential toilet flushing, swimming pools, hot tubs, spas, gardens containing food crops, or any other use with potential for direct contact with the reclaimed water. 22. The Permittee is responsible for the operation and maintenance of the entire treatment and reuse system including the following items: a. The septic tank shall be checked annually and pumped out as needed.. The septic tank effluent filter shall be checked and cleaned at the same frequency as the septic tank. b. The ultraviolet disinfection unit shall be checked weekly. The lamp(s) and quartz sleeve(s) shall be cleaned or replaced as needed to ensure proper disinfection. c. The storage, pump, and alarm systems shall be inspected monthly. The scum layer shall be removed at the same interval as the septic tank is cleaned out. d. The system shall be inspected monthly to make certain of the proper operation of the spray heads, that the system is free of leaks, that the vegetative growth allows a proper spray pattern, that the soil is assimilating the disposed treated wastewater with no surface runoff, and that no objectionable odors are being generated. IV. MONITORING AND REPORTING REQUIREMENTS I. Any Division required monitoring (including groundwater, plant tissue, soil and surface water analyses) necessary to ensure groundwater and surface water protection shall be established, and an acceptable sampling reporting schedule shall be followed. 2. Per 15A NCAC 02H .0800, a Division certified laboratory shall conduct all laboratory analyses for the required effluent, groundwater or surface water parameters. 3. Flow through the reclaimed water generating facility shall be continuously monitored, and daily average flow values shall be reported on Form NDMR. 4. The Permittee shall monitor the reclaimed water from the generating facility at the frequencies and locations for the parameters specified in Attachment A. 5. The Permittee shall maintain adequate records tracking the amount of reclaimed water utilized for a minimum of five years. These records shall include the following information for the utilization site Iisted in Attachment B: a. Date of reclaimed water utilization; b. Length of time site is irrigated; c. Continuous monthly and year-to-date hydraulic (inches/acre) loadings; d. Maintenance of cover crops. 6_ A record shall be maintained of all residuals removed from this facility for a period of no less than five years, and shall be made available to the Division upon request. A maintenance log shall be maintained at this facility for a period of no less than five years, and shall be made available to the Division upon request. At a minimum, this log shall include: a. Visual observations of treatment plant and plant site and utilization areas; b. Record of preventative maintenance and all repairs; and c. Record of all unpermitted releases of reclaimed water to surface water or land surface including date of occurrence, estimated volume of release, cause, and corrective action taken. Three copies of all effluent monitoring data shall be submitted on Form NDMR for each. PPI listed in Attachment A. Reporting fortes shall be submitted on or before the last day of the following month. If no reclaimed water distribution activities occurred during the monitoring month, monitoring reports documenting the absence of the activity are still required to be submitted. All effluent monitoring data shall be submitted to the. following address: Division of Water Resources, Information Processing Unit, 1617 Mail Service Center, Raleigh, North Carolina, 27699-1617. 9. Three copies of all operation and utilization records shall be submitted on Form NDAR- I for every non -conjunctive utilization site listed in Attachment B. Reporting forms shall be submitted on or before the last day of the following month. If no reclaimed water utilization activities occurred during the month, monitoring reports are still required documenting the absence of the activity. All information shall be submitted to the following address: Division of Water Resources, Information Processing Unit, 1617 Mail Service Center, Raleigh, North Carolina, 27699-1617. 10. Noncompliance Notification: The Permittee shall report by telephone to the Raleigh Regional Office, telephone number (919) 791- 4200, as soon as possible, but in no case more than 24 hours, or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any occurrence at the facility resulting in the treatment of significant amounts of wastes that is abnormal in quantity -or characteristic, including the known passage of a hazardous substance. b. Any process unit failure (e.g., mechanical, electrical, etc.), due to known or unknown reasons, rendering the facility incapable of adequate wastewater treatment. c. Any failure resulting in a discharge of reclaimed water directly to surface waters or any unperrnitted release of reclaimed water to land surface greater than or equal to 5,000 gallons. Unpermitted releases less than 5,000 gallons to land surface shall be documented by the permittee in accordance with Condition IV.7 but do not require Regional Office notification. Ike I d. Any time self monitoring indicates the �cilities permitted herein have gone out of compliance with the limitations contained in this perrlxit. i c. Ponding in or runoff from the reclaimed water utilization sites. Any emergency requiring immediate reporting (e.g., discharges to surface waters, imminent failure of a storage structure, etc.) outside normal business hours shall be reported to the Division's Emergency Response personnel at telephone number (800) 662.7956, (800) 858-0368, or (919) 733-3300. Persons reporting such occurrences by telephone shall also file a written report in letter form within five days following first knowledge of the occurrence. This report shall outline the actions taken or proposed to be taken to ensure the problem does not recur. WQ0036557 Version 1.0 Shell Version 131016 Page 7 of 10 V. MSPECTIONS 1. The Permittee shall provide adequate inspection and maintenance to ensure proper operation of the reclaimed water generation and utilization facilities. 2. The Permittee or their designee shall inspect the reclaimed water generation and utilization facilities to prevent malfunctions, facility deterioration and operator errors resulting in discharges, which may cause the release of wastes to the environment, a threat to human health or a public nuisance. The Permittee shall maintain an inspection log that includes, at a minimum, the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken. The Permittee shall maintain this inspection log for a period of five years from the date of the inspection, and this log shall be made available to the Division upon request. 3. Any duly authorized Division representative may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the reclaimed water generation and utilization facilities permitted herein at any reasonable time for the purpose of determining compliance with this permit;. may inspect or copy any records required to be maintained under the terms and conditions of this permit, and may collect groundwater, surface water or leachate samples. VI. GENERAL CONDITIONS 1. Failure to comply with the conditions and limitations contained herein may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statutes 143- 215.6A to 143-215.6C. 2. This permit.shall become voidable if the permitted facilities are not constructed in accordance with the conditions of this permit, the Division approved plans and specifications, and other supporting documentation. 3. This permit is effective only with respect to the nature and volume of wastes described in the permit application, Division approved plans and specifications, and other supporting documentation. No variances to applicable rules governing the construction or operation of the permitted facilities are granted, unless specifically requested and approved in this permit pursuant to 15A NCAC 02T .0105(n). 4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other jurisdictional government agencies (e.g., local, state, and federal). Of particular concern to the Division are applicable river buffer rules in 15A NCAC 02B .0200; erosion and sedimentation control requirements in 15A NCAC Chapter 4 ! and under the Division's General Permit NCG0100100; any requirements pertaining to wetlands under ! 15A NCAC 02B .0200 and 02H .0500; and documentation of compliance with Article 21 Part 6 of Chapter 143 of the General Statutes. 5. In the event the permitted facilities change ownership or the Permittee changes their name, a formal permit modification request shall be submitted to the Division. This request shall be made on official Division forms, and shall include appropriate property ownership documentation and other supporting documentation as necessary. The Permittee of record shall remain fully responsible for maintaining and operating the facilities permitted herein until a permit is issued to the new owner. 6. The Permittee shall retain a set of Division approved plans and specifications for the life of the facilities permitted herein. WQ0036557 Version 1.0 Shell Version 131016 Page 8 of 10 7. The Permittee shall maintain this permit until all permitted facilities herein are properly closed or permitted under another permit issued by the appropriate permitting authority pursuant to 15A NCAC 02T .01050). 8. This permit is subject to revocation or unilateral modification upon 60 days notice from the Division Director, in whole or part for the requirements listed in 15A NCAC 02T .0110. 9. Unless the Division Director grants a variance, expansion of the permitted facilities contained herein shall not be granted if the Permittee exemplifies any of the criteria in 15A NCAC 02T .0120(b). 10. The Permittee shall pay the annual fee within 30 days after being billed by the Division. Owner agrees to operate and maintain the wastewater treatment and irrigation system in accordance with the following: a) All treatment tank compartments shall be checked annually and pumped out as needed per a service provider's inspection. The service provider should check for leaks and measure the scum and sludge layers in all tank chambers. Chambers shall be pumped when: the scum layer is 3 inches from the bottom of the outlet tee, the sludge layer is 9 inches from the bottom of the outlet tee, or if the sludge and scum layers combined equal 25-33% of the liquid capacity of any chamber. b) The treatment tank effluent filter shall be cleaned as needed at the beginning of operations in order to monitor the amount of clogging particles that are found in the treatment tank. An inspection schedule should be kept based on initial inspections. Effluent filters should be cleaned over the access to the aerobicibuffer chamber, allowing discharge to flow back into the treatment tank. c) The membrane bioreactor system shall be operated and serviced per instructions provided by A3 - USA, Inc. d) The UV system shall be operated and serviced by the ORC for this facility during the non -nal maintenance cycle. e) All pump and alarm systems shall be inspected every six months, including float function. f) The irrigation system shall be inspected every six months to ensure that the system is free of leaks, that all irrigation equipment is operating as designed, that vegetative growth does not interfere with the system inspection or operation, that the soil is assimilating the reclaimed water with no visible runoff or ponding, and that na objectionable odors are being generated. , l g) The cover crop Fescue will be maintained by the owner through mowing and clearing of buildup of debris. It should be overseeded as needed in the fall or spring for thicker sod formation and to repair thinning areas. h) Ensure landscaping/grading is designed and maintained in order to minimize the amount of rainwater runoff entering the spray field area. i) Access routes for all vehicles, septic pump trucks, and other maintenance vehicles should be clearly marked on the ground or given to the drivers to ensure vehicles are not traversing shallow sections of plastic sewer and water pipe or damaging access risers and cleanouts for the wastewater treatment and irrigation system.
