In accordance with a written request received on May 18, 2011 from Matthew Fleahman of the Raleigh
Regional Office's Aquifer Protection Section, we are forwarding herewith a corrected Attachment A for
Permit No. WQ0033406, issued May 26, 2009.
The following corrections have been made to the permit:
1. The requirement to monitor parameters Total Dissolved Solids (70300) and Chloride (00940) has
been removed,
2. All previous sample types listed as "Composite" have been changed to "Grab."
3. The requirement to continuously monitor and record effluent flows has been changed to a monthly
estimate.
4. The measurement frequency for pH has been reduced from "5 x Week" to "Weekly."
Please replace Attachment A with the corrected Attachment A into Permit No. WQ0033406, issued
May 26, 2009.
