May 19, 2016. However, additional information is required before the review may be completed. Please
address the items on the attached pages no later than the close of business on June 24, 2016.

regulations. Any oversights that occurred in the review of the subject application package are still the
Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in

A. Ownership of the Facility:
1. The Warren County Tax Records indicate that the site is owned by Smithfield Carroll's Farms.
This business is not registered with the State of North Carolina's Secretary of State's Office. Our
agency is not allowed to issue a permit to a business which is not registered with the State of North
Carolina's Secretary of State's Office. A property deed is needed which confirms ownership of the
facility by a business that is registered with the State ofNorth Carolina's Secretary of State's Office.
Please submit a deed of property ownership confirming that the property at 846 Jessie Shearin Road
in Vaughan is owned by a business that is registered with the State of North Carolina's Secretary
of State's Office. If the owner of the property is an entity other than Murphy -Brown, LLC, please
also submit a completed permit renewal form (Form NDSR 11-13) with the name of the business
as the Permittee's name and signed by a person who is a Vice President or higher.
