The following corrections have been made to the permit: Condition III 7 has been corrected to indicate that use of Form IRR-2 shall be necessary for reporting monthly plant available nitrogen loadings. Please replace Page 5 with the corrected Page 5 into Permit No. WQ0016376, issued July 3, 2003. Adequate records shall be maintained by the Permittee tracking the amount of wastewater disposed. These records shall include, but are not necessarily limited to, the following information: a. date of irrigation, b. volume of wastewater irrigated, c. field irrigated, d. length of time field is irrigated, e. continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each field, f. continuous weekly, monthly, and year-to-date plant available nitrogen (PAN) (inches/acre) loadings for each field, g. weather conditions, and h. maintenance of cover crops. 6. Freeboard in the lagoon shall be recorded at least weekly. 7. Three (3) copies of all monitoring data (as specified in condition III 2, III 3 and I114) on Form NDMR-1 and three (3) copies of all operation and disposal records (as specified in condition III 5, and III 6) on Form NDAR-1 and IRR-2 (for plant available nitrogen loadings) shall be submitted monthly on or before the last day of the following month. All information shall be submitted to the following address: NC Division of Water Quality Water Quality Section Non -Discharge Compliance/Enforcement Unit 1617 Mail Service Center Raleigh, North Carolina 27699-1617 A representative annual soils analysis (Standard Soil Fertility Analysis) shall be conducted on each irrigation field and the results maintained on file by the Permittee for a minimum of five years. The Standard Soil Fertility Analysis shall include, but is not necessarily limited to, the following parameters: Acidity Manganese Potassium Calcium Percent Humic Matter Sodium Copper pH Zinc Magnesium Exchangeable Sodium Percentage Phosphorus Cation Exchange Capacity Base Saturation (by calculation) 9. An annual report shall be submitted to the Division, on or before March I" of the following year, outlining the operation and maintenance techniques used to prevent nuisance conditions. This report shall be submitted to the Division of Water Quality, Wilmington Regional Office, 127 Cardinal Drive Extension, Wilmington, NC 27889 and shall include, but not be limited to: Operation and Maintenance Schedule(s) Equipment repair and replacement Odor Control measures Complaints lodged by citizens 10. Noncompliance Notification: Annual Soils Analysis Lime Application Records, if applicable Cover Crop Harvest Dates & Yields Other Field Maintenance Activities The Permittee shall ,report by telephone to the Wilmington Regional Office, telephone number (910) 395-3900, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following:
