This certification is being issued to modify Attachment B by: Transfer of nine fields in Davie County totaling 119.85 acres consisting of two fields belonging to Cletus Miller (NC-DV-192-1 and -3) totaling 54.20 acres and seven fields belonging to Edward H. C. Ellis and George H. Leagans (NC-DV- 125-5, -6, -7, -8, 23, -40, and -41) totaling 65.65 acres. This modified Attachment B shall be effective from the date of certification until December 31, 2016 (i.e. expiration date of the current permit); shall void Attachment B of Permit No. WQ0010583 that was previously certified on January 21, 2008, and shall be subject to the conditions and limitations as specified therein and well as those specified in the above -referenced permit. Please replace the Attachment B of Permit No. WQ0010583 that was issued on January 21, 2008 with this modified Attachment B. Please note that only the maps for any new residuals -generating sources and/or Iand application sites are attached to this letter. It is the Permittec's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements, and/or limitations contained in this certification or modified Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit shall be final and binding.
