Dear Mr. Hudgens:
In response to information received on December 5, 2005, regarding the modified attachment B dated
December 1, 2005,. we are forwarding herewith a corrected Attachment B, certified December 6, 2005 to Delta
Apparel, Inc., for the subject residuals land application program.
Please replace the existing Attachment B of Permit No. W00006984 that was previously certified
on December 1, 2005, with this modified Attachment B. Note that the vicinity and buffer maps for any new
and modified land application sites are the same as before. Only the conditions have been changed/corrected.
It is the Permittee's responsibility to ensure inserting this modified Attachment B into the subject permit and
creating a complete permit.
This modified Attachment 6 shall be effective from the date of certification; shall void Attachment B of
Permit No. WQ0006984 that was previously certified on December 1, 2005, and shall be subject to the
conditions and limitations as specified therein and well as those specified in the above -referenced permit.
If any parts, requirements, and/or limitations contained in this certification or modified Attachment B are
unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days
following receipt of the certification. This request shall be in the from of a written petition, conforming to
Chapter 150E of the North Carolina General Statutes and filed with the Office of Administrative Hearings at
6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit
shall be final and binding.
