Spray Irrigation System Permit
Dear Mr. Sawyers: ,
Reference is made toward expiration of the subject Spray Irrigation System Permit.
Staff of our Winston-Salem Regional Office have confirmed that this NonDischarge =_
Permit is no longer required. The .permit is rescinded and is being removed from our
computer systems, effective immediately.
If in the future you wish to again operate under a nondischarge wastewater treatment
system, you must first apply for and receive a new State Permit. Operating without a
valid State Permit will subject the facility to a civil penalty of up to $10,000 per day.
Within 60 days of permit rescission, the four (4) existing monitoring wells must be
abandoned in accordance with 15A NCAC 2C .0113 (Abandonment of wells). A well
abandonment record (form GW-30, attached) must be submitted to the Winston-Salem
Regional Office upon abandonment of the monitoring wells.

Lagoon/Spray Irrigation System
Forsyth County
The Groundwater Section has reviewed the request by the permittee to rescind the
permit and to discontinue groundwater monitoring. The permittee has stated that the
lagoon/spray irrigation facility is no longer in operation, since the facility was hooked up
to a forced main sewer, in October 1995. Having reviewed the compliance monitoring data
(no exceedances of groundwater standards), we concur with comments from the Regional
Office (Groundwater Section and Water Quality Section), that groundwater monitoring is
no longer necessary at this site and recommend that the following condition be included:
*1. Within 60 days of permit rescission, the four (4) existing monitor wells must
be abandoned in accordance with 15A NCAC 2C .0113 (Abandonment of
Wells). A Well abandonment record (form GW-30, attached) must be
submitted to the Winston-Salem Regional Office upon abandonment of the
monitor wells.
