Dear Mr. Williams:
In accordance with your permit modification application package received on, July 20, 2005, we
are forwarding herewith a modified Attachment A, certified October 14, 2005, to Town of Yadkinville for
the subject residuals land application program.
This certification is being issued to modify Attachment A to approve residuals from one
additional residuals source -generating facility for land application under this residuals Iand application
program. Up to 30 dry tons of alum residuals from the Town of Yadkinville Water Treatment Plant are
approved for land application as liquid, separately from the WWTP residuals.
Please replace the existing Attachment A of Permit No. WQ0001800 that was previously
certified on September 26, 2003 with this modified Attachment A. Note that only the vicinity maps
for any new and modified residuals source -generating facilities are attached to this modified Attachment
A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this
modified Attachment A and vicinity map into Permit No. WQ0001800, issued on September 26, 2003,
and making sure to retain any vicinity maps for any existing and unmodified residuals source -generating
facilities.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQOOO 1800 that was previously certified on September 26, 2003; and shall
be subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. As always, remember to take the time to review this modified Attachment A
thoroughly, as some of the conditions contained therein may have been changed since the last
certification.

Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.
