Dear Mr. Hudson:
Reference is made to your request for rescission of the subject Surface Disposal of Residuals Permit. The
Division has received the Well Abandonment Record which fulfilled the requirements specified on our
previous letter dated August 22, 2012. Therefore, in accordance with your request, Permit No.
WQ0001759 is rescinded, effective immediately.
If in the future, you wish again to operate a non -discharge system, you must first apply for and receive a
new permit. Operating a non -discharge system without a valid permit will subject the responsible party to
a civil penalty of up to $25,000 per day.
