Elon College, NC 27244 SUBJECT: Permit No, WQ0001147 Ms. Annette Whitman Spray Irrigation Wastewater Treatment and Disposal Facility Caswell County Dear Ms. Whitman: In accordance with your application received February 1, 1989, we are forwarding herewith Permit No. WQ0001147, dated March 6, 1989, to Annette Whitman for the construction and operation of the subject spray irrigation wastewater treatment and disposal system. This permit shall be effective from the date of issuance until March 31, 1994, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming.to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative hearings, Post Office Drawer 1.1666, Raleigh, North Carolina 27604. Unless such demands are made this permit: shall be final and binding. One (.1.) set of approved plans and specifications is being forwarded to you. P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Annette Whitman Caswell County FOR THE construction and operation of a 450 GPD spray irrigation wastewater treatment and disposal facility consisting of two 1,040 gallon septic tanks, two subsurface and filters (255 square feet and 150 square 'Feet), tablet chlorinator, a 2,400 gallon storage/chlorination/pump tank with a single 1.5 GPM submersible pump, and a 12,868 square foot spray irrigation and disposal area to serve the Whitman residence, pursuant to the application received February 1, 1989, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until March 31, 1994, and shall be subject to the following specified conditions and limitations.. 1. This permit shall become voidable unless the :facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. 5. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the requirements of this permit and the approved plains and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687. 6. A set of approved plans and specifications for the subject project must be retained by the applicant for the life of the project. 7. The spray irrigation facility'shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will. render them unsatisfactory for normal. use. :In the event that the fac'.1itiez f.r il. to perform satisfactorily, in,lluding the creation of nuisance conditions or failure of the irrigation area to adequately absorb the wastewater, the Permittee shall take such immediate corrective action as may be required by the Division of Environmental Management. 8. The Winston Salem Regional Office, phone no. 919/761-2351 shall be notified at least twenty-four (24) hours in advance of backfilling of the installed spray irrigation facility so that an in-place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 9. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approvable by the Forth Carolina Division of Environmental Management. 10. A suitable year round vegetative cover shall be maintained. 11. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 12. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 13. The application rates shall not exceed 22 inches/year or 0.4 inches/week. 14. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 15. The facilities shall. be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 16. No type of wastewater other than that from the Whitman residence shall be sprayed onto the irrigation area. 17. No traffic or equipment shall be allowed on the disposal. area except while installation occurs or while normal maintenance i.s being performed. 18. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 19, In any future transfer of this land, a notice shall be given to the new owner that gives full details of the materials applied or incorporated at this site. 20. The Permittee is responsible for the operation and maintenance of the entire treatment and disposal system inclildi.ng, but not limited to, the following items: s. The ^.optic tank shall, be checked every sly. (6" months sinci dumped out as required. b. The tablet chlorinator shall be checked eti-ery week until the actual. usage rate is established and thence on a monthly basis. Tablets shall be added so as to provide proper chlorination. c. The storage system, pump system, and alarm system shall be inspected monthly. The scum layer shall be removed at the same interval as the septic tank is cleaned out. d. The spray system shall be inspected monthly to mare certain of the sprayers proper operation, that the vegetative growth allows proper spray patterns, that the soil is absorbing -the disposed treated wastewater with no surface runoff, and that no objectionable odors are being generated. 2.1. Failure of the Permittee to properly operate this system is subject to a penalty tap to $10,000 per day. 22. Failure to meet permit conditions or violation of the State's surface or groundwater regulations will require cessation of occupancy of the residence. 23. The sand media of the spray irrigation facility must comply with the Division's specifications. The engineer's certification will serve as proof of this compliance. 24. Any additional groundwater quality monitoring as deemed necessary by the Department, shall be provided. 25. This permit may be voided if an operation and maintenance agreement is not signad by the Permittee and a signed copy returned to this Division. 26. The septic tank, sand filter, and irrigation field shall be located. at least 100 feet away from any drinking water well. 27.
