Wastewater Treatment and Spray Irrigation Duplin County Dear Mr. McIntyre: In accordance with your application received July 25, 1988, we are forwarding herewith Permit No. WQ0000884, dated December 7, 1988, to Carolina Turkeys, Inc. for the continued operation of the subject wastewater treatment and disposal facilities. This permit shall be effective from the date of issuance until December 31, 1989, and shall void Permit No. 12496, and shall be subject to the conditions and limitations as specified therein. Please not that the acreage associated with Zone 11 of the spray irrigation disposal field has been deleted from this permit as was requested. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall be final and binding. Duplin County FOR THE continued operation of a 1.5 MGD wastewater treatment and spray irrigation disposal facilities consisting of a 15 million gallon aerated lagoon; a 1.85 MGD capacity '-imella clarifier; an anaerobic digestor; a 36,400 -gallon per day pretreatment .tended aeration -type package treatment plant for sanitary waste including a bar screen, a 10,000 -gallon aerated flow equalization tank, a 36,400 -gallon aeration tank, dual aeration blowers, an aerobic digestor, a 6,000 gallon clarifier, and a chlorine contact chamber and feed system, a 41 million gallon holding lagoon (lagoon no. 2) and approximately 613 acres of spray irrigation area; and all associated piping, valves and appurtenances to serve the Carolina Turkeys Plant in Duplin County with no discharge of wastewater to the surface waters of the State, pursuant to the application received July 25, 1988, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until December 31, 1989, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. 4 5. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and ground waters. 6. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 7. The Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 8. Diversion or bypassing of the untreated wastewater from the land application facilities is prohibited. 9. No wastewater shall be applied within 400 feet of an occupied dwelling, within 150 feet of adjoining property, or within 100 feet of surface water. The sprinklers in Zone 6B, which are adjacent to the plant building, shall be operated from midnight to 6:00 a.m. only. 10. Wastewater shall not be disposed of within areas prone to flooding by the 100 -year storm. 11. Solids, sludges, or other pollutants removed or resulting from the wastewater storage .facilities shall be contained and disposed of in such a manner as to prevent any contamination of the surface or groundwaters of the State. If sludge is to be land applied, a separate non -discharge permit must be obtained from the Division of Environmental Management. 12. No wastewater small be hauled unless permission is obtained from the Division's Wilmington Regional Office. 13. Wastewater shall not be applied during inclement weather or within 24 hours following a rainfall event of 1/2 inch or greater in 24 hours. Emergency sludge disposal measures shall be approved by the Division of Environmental Management. 14. The issuance of this permit shall not relieve Carolina Turkeys, Inc. of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 15. In the event odor problems occur, immediate action, including further pretreatment such as may be required by this Division, shall be taken by the Permittee. 16. A suitable year round vegetative cover shall be maintained. 17. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. No root or leaf crops for public consumption shall be raised on these sites. 19. All spraying of treated wastewater must be done within the designated and protected spray irrigation fields as indicated on the approved engineering plans. 20. Lagoon No. 2 shall be constructed with a compacted clay liner at least one foot thigk at all locations and with a maximum permeability not to exceed 1 x 10 cm/sec. Following installation and inspection of the lagoon liner and prior to waste treatment operations, verification of the liner's compliance with the permeability and thickness requirements must be provided to the Department by the project engineer. 21. The Permittee shall provide written notification (within 30 days of start-up) to the Groundwater Section, Wilmington Regional Office, stating the date that lagoon No. 2 was placed into operation. 22. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. 23. All sludges removed from the domestic wastewater treatment facility shall be disposed of in a manner acceptable to the Division. A septage hauler cannot lawfully dispose of this sludge on sites approved by DHS for septage disposal. 24. The operator shall maintain a daily log providil.g, at a minimum, the following information. a. Date, time, and volume of wastewater applied to ear:h zone. b. Climatic information including temperatures high and low, rainfall amount, time of rainfall event, wind velocity and direction. 25. No irrigation above the amount necessary to establish a cover crop shall take place on the insufficiently covered areas of Zones 9A, 9B, 9D, and 10C. 26. Within 90 days of permit issuance, two (2) monitor wells, one downgradient of the holding lagoon site, and one downgradient and outside of the perimeter of the recently permitted (March 16, 1988) spray field, shall be installed installed to monitor groundwater quality. The location and construction details for these wells must be by approval of the Wilmington Regional Office, from which a well construction permit must be obtained. 27. The permittee shall sample seven (7) monitor wells; MW -9 (upgradient well), M4-14, MW -8, MW -11, MW -12, the new monitor well downgradient of the lagoon site and the new well downgradient of the recently permitted spray field. These wells must be sampled every March, duly and November for the following parameters: NO pH ToLl Ammonia TOC TDS Chloride Water Levels Total Coliforms Fecal Coliforms Orthophosphate TOX (in Nov. only) Sodium The two new monitor wells must also be sampled immediately after construction for the parameters specified above. The measurement of water level must be made prior to sampling for the remaining parameters. The analytical methods used for TOX must be capable of detecting the total of all halogenated organic compounds present at a concentration of 5.0 parts per billion (ppb) or greater. In the event that a TOX concentration of 5.0 ppb or greater is detected, any individual halogenated organic compound(s) present at a concentration at or above the method detection limit (MDL) must be identified and quantified utilizing BPA methods 601, 602, 604 and 611. The results of the sampling; and analysis shall be sent to the N.C. Division of Environmental Management on Form GW -59 (Compliance Monitoring Report Form) every April, August and December.
