In accordance with your application received November
29, 1988, we are forwarding herewith Permit No. WQ0000843,
dated January 9, 1989, for the construction and operation of
the subject nondischarge waste treatment facilities.
This Permit shall be effective from the date of
issuance until rescinded, and shall be subject to the
conditions and limitations as specified therein.
If any parts, requirements, or limitations contained
in this Permit are unacceptable to you, you have the right
to an adjudicatory hearing before a hearing officer upon
written demand to the Director within 30 days following
receipt of this Permit, identifying the specific issues to
be contended. Unless such demand is made, this Permit shall
be final and binding.

For the Disposal of Sewage, Industrial Wastes, or Other Wastes
in accordance with the provisions of Article 21 of
Chapter 143, General Statutes of North Carolina as amended,
and other applicable Laws, Rules and Regulations
PERMISSION IS HEREBY GRANTED TO

For The
construction and operation of a 100 gallon per day septic
tank system, consisting of a new 1500 gallon dual
compartment baffled and teed septic tank, to be placed
up -line of an existing 1500 gallon single compartment septic
tank with existing 1200 square feet of drain field, to serve
his automotive shop, with no discharge of wastewater to the
waters of the State, pursuant to the application received
November 29, 1988, and in conformity with the project plans,
specifications and other supporting data, subsequently filed
with and approved by the Department of Natural Resources and
Community Development and considered a part of this Permit.
This Permit shall be effective from the date of
issuance until rescinded, and shall be subject to the
following specified conditions and limitations:
1. This Permit shall become voidable unless the facilities
are constructed in accordance with the approved plans,
specifications and other supporting data and are
completed and placed in operation on or before January
9, 1990, or as this date may be amended.

2. This Permit is effective only with respect to the
nature and volume of wastes described in the
application and other supporting data.
3. This Permit shall become voidable in the event of
failure of the soil to adequately absorb the wastes,
and may be rescinded unless the facilities are
installed, maintained and operated in a manner which
will protect the assigned water quality standards of
the surface waters, and prevent any contamination of
the ground waters which will render them unsatisfactory
for normal use.
4. In the event the facilities fail to perform
satisfactorily, the permittee shall take such immediate
corrective action as may be required by this
Department.
5. Staff of the Division of Environmental Management at
the Winston-Salem Regional Office, 8025 North Point
Boulevard, Winston-Salem, North Carolina 27106
(telephone No. 919/761-2351) shall be notified at least
twenty-four (24) hours in advance of backfilling of the
installed subsurface disposal system so that an
in-place inspection can be made of said system prior to
backfilling. Such notification shall be made to the
Regional Supervisor during the normal office hours from
8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.
6. This Permit is not transferable.
7. The method for final disposal of screenings, scum and
sludges from these facilities shall not result in any
contamination of the surface or ground waters of the
State.
8. Sufficient land area shall be reserved for a complete
drain field should the original field fail.

9. Approved nitrification field site shall not be
disturbed, regraded or substituted with out written
permission from the Winston-Salem Regional Supervisor.
