Box 20 Danbury, N.C. 27016 SUBJECT: Permit No. WQ0000556 Stokes County Board of Commissioners Stokes County Government Center Pump and Haul Wastewater Disposal Stokes County Dear Mr. Carroll.: In accordance with your application received September 27, 1988, we are forwarding herewith Permit No. WQ0000556, dated October 28, 1985, to the Strokes County Hoard of Commissioners for the operation of the subject pump and haul activities to serve the Danbury Library of the Stokes County Government Complex. This permit shall be effective from the date of issuance until July 1, 1989, and shall be subject to the conditions and ].imitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request ail adjudicatory hearing upon written request within 30 days following receipt of this permit, This .request must be in the form of a written petition, conforming to Chapter 150E of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919.733-7015 An Equal Opportunity Affirmative Action Employer NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sawage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO The Stokes County Board of Commissioners Stokes County FOR THE operation of a 250 GPD pump and Haul wastewater disposal system to transport wastewater from an existing failing subsurface facility serving the Danbury Library, Stokes County Government Complex to the Town of Walnut y.ove`s existing sewerage system, pursuant to the application received September 27, 1988, and in conformity with the supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until July 1, 1989, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are operated in accordance with the approved supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. The sewage and wastewater collected by this system shall be treated in the Town of Walnut Coves Wastewater Treatment Plant prior to being discharged into the receiving stream. 5. This permit is not transferable. 6. This permit shall become voidable unless the agreement between the Town of Walnut Cove and the Stokes County Board of Commissioners for the collection and final treatment of wastewater is in full force and effect. 7. This permit shall become voidable unless the agreement between the Stokes County Board of Commissioners and Mr. Bobby Joe Mabe of Mabe Septic Tank Pumping Service for the transportation of wastewater is in full force & effect. 8. In the event that the facilities fail to perform satisfactorily, including the creation -of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 9. The Stokes County Board of Commissioners is liable for any damages caused by a spill or a failure of the pump and haul operations. 10. No type of wastewater other than that from the Danbury Library, Stokes County Government Complex shall be included in these pump and haul operations. I.J.. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 12. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 13. An accurate record must be maintained by the Permittee including: a) Date wastewater was pumped b) Volume of wastewater pumped c) Receipt from the Town of Walnut Cove These records shall be submitted on the 10th day of every month to the Winston-Salem Regional Office, 14. This operation shall be checked daily by the permittee or a representative for compliance with all terms & conditions of this permit. 15. This permit shall become voidable unless the performance bond secured through the U.S. Insurance Group is maintained through July 1, 1989 or until permanent cessation of these pump and haul operations occurs. 16.
