Therefore, these fields shall receive residuals only during the months of April through November inclusive. ° Special caution should be used in the management of site 92-01 (McAdams farm). If residuals are applied to the farm under current conditions, the contractor is urged to make applications in a conservative manner such that potential for runoff is precluded. A preferred method involves the utilization of a pasture management program which would reduce weed cover, maintain the soil at an ideal pH, and supply proper nutrients to the grass to establish a suitable sod for future residual application. In addition, the land application area covered with numerous boulders and bedrock outcrops, approximately one acre, should be deleted (see attached map). Due to massive clays at shallow depths and/or wet soils, no residuals shall be applied to these sites during the period from December through March, inclusive. d Sludge shall not be applied to the Helena soils. e Sludge shall not be applied to the Wilkes soils. The barn located in this field shall be buffered by 15 feet. 9 A 50 foot buffer will be maintained from the low wet area in this field. h Site is approved provided the stream crossing has a culvert installed prior to any sludge application. The Winston-Salem Regional`Office shall be notified 24 hours prior to application. I The mobile homes shall be buffered with the appropriate buffer. 14 J Fields 64-08 and 64-1 :_r:: -$fists of areas cZ'weathered rock soils at 2 feet below land surface. Therefore, these fields shall receivy sludge at 1/2 normal application rates and only during the months of April through November inclusive. k These land application sites are covered in part by soils having a perched/seasonal high water table at depths of less than three (3) feet below land surface. Therefore, these fields shall receive sludge at 1/2 normal application rates and only during the months of April through November inclusive. t A 25 ft. buffer must be maintained around the drainage way m These land application sites are covered in part by soils having a perched/seasonal high water table at depths of less than three (3) feet below land surface. Therefore, these fields shall receive residuals only during the months of April through November inclusive. n Special caution should be used in the management of site 92-01 (McAdams farm). 5. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division of Environmental Management in accordance with North Carolina General Statute 143-215.6(x) to 143-215.6(c). 6. The annual administering and compliance fee must be paid by the Permittee within thirty (30) days after being billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit as specified by 15 NCAC 2H .0205 (c)(4). 7. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 8. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. 9. This permit may be modified, or revoked and reissued to incorporate any conditions, limitations and monitoring requirements the Division of Environmental Management deems necessary in order to adequately protect the environment and public health. 10. This permit shall become voidable unless the agreements between the Permittee and the Ian downers/les sees are in full force and effect. The land owner agreements are considered expired concurrent with the expiration date of the permit and must be renewed at the same time the permit is renewed. 15 Permit issued this the 13th ,?•ay ^f December, 1993 NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION km 01 ' A.
