In accordance with your permit modification application package first received on October 11, 2002
as well as additional information received on October 30, 2003, we are forwarding herewith a modified
Attachment B, certified on February 2, 2004, to the City of Burlington for the subject residuals land
application program.
This certification is being issued to modify Attachment B by adding 8319 acres to the City of
Burlington Residuals Land Application Program. The initially requested 896.7 acres was reduced to 831.9
acres, due to the addition of new buffers after inspection of the proposed sites. Please note changes in site
numbering since the last certification. Also, the net acreage has been updated for many of the previously
approved sites. Finally, please note that some sites have been deleted, as they have been dropped from the
residuals program.
Please replace the existing Attachment B of Permit No. WQ0000520 that was previously
certified on June 6, 2001, with this modified Attachment B. Note that the vicinity and buffer maps for
any new and modified land application sites are attached to this modified Attachment B. It is the
Permittec's responsibility to ensure that a complete permit is created by inserting this modified Attachment
B into Permit No. WQ0000520, issued June 6, 2001, and making sure to retain any vicinity and buffer
maps for any existing and unmodified land application sites.
This modified Attachment B shall be effective from the date of certification; shall void Attachment
B of Permit No. WQ0000520 that was previously certified on June 6, 2001; and shall be subject to the
conditions and limitations as specified therein as well as those specified in the above referenced permit. As
always, remember to take the time to review this modified Attachment B thoroughly, as some of the
conditions contained therein may have been changed since the last certification. Note that this modified
Attachment B is hereby incorporated by reference into Permit No. WQOOOO520, issued on June 6, 2001. If
any parts, requirements, and/or limitations contained in this certification or modified Attachment B are
unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days
following receipt of the certification.

This request shalir be in the Foran of a written petition, conforming to Chapter 150B of the North

'A 25 -foot buffer shall be maintained around the drainage way,

`This site is dominated by soils or site conditions which readily produce perched water table conditions, or have very shallow depths to bedrock.

These land application sites are covered in part by soils having a perched/seasonal high water table al depths of less than three feet below land surface.
These fields, therefore, shall receive residuals at half the normal application rates, and only during; the months of April through November, inclusive.

The City of Burlington requests that 896.7 acres be added to the City's land application program.
WSRO staff (Jenny Freeman, Mike Mickey, and Corey Basinger, WQ, Chris Greene, GW, and
Perry Wyatt, DSWC) visited these sites on October 13 and I4, 2003 with Jeff Spivey, Synagro
South, Inc. Technical Services Manager. These eleven sites (59 fields) are in five different
counties. The sites are listed in the following table with comments from the site inspections.

The additional buffers that Synagro added following WSRO comments during the site inspections
bring the total acres from 896.7 to 831.9. With the addition of these buffers, WSRO concurs that
this City of Burlington permit modification is acceptable and concurs with its issuance.
