Rockingham County Dear Mr. Johnston: In accordance with your application received September 22, 1988, we are forwarding herewith Permit No. WQ0000497, dated October 31, 1988, to 220 Trucking Company, Incorporated for the construction and operation of the subject non -discharge wastewater disposal system. This permit shall be effective from the date of issuance until October 31, 1993, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. If you wish to have the hearing before the Hearing Officer with this Department, you must indicate in the petition that you waive the right to have the contested case conducted by a Hearing Officer in the office of Administrative Hearings, and wish to have the matter conducted in the Department of Natural Resources and Community Development. Winston-Salem Regional Office Dennis Ramsey NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules and Regulations PERMISSION IS HEREBY GRANTED TO 220 Trucking Company, Inc. Rockingham County FOR THE construction and operation of a 300 GPD subsurface wastewater disposal system consisting of a 300 gallon precast grease trap, 1000 gallon septic tank, precast concrete distribution box and a nitrification field consisting of four (4) drain lines (each 90 feet long, spaced 10 feet apart) to serve 220 Trucking Company with no discharge of wastewater to the surface waters of the State, pursuant to the application received September 22, 1988, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until October 31, 1993, and shall be subject to the following specified conditions and limitations. 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. With no discharge of wastewater to the surface waters of the State. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properlymaintained and operated at all times. 4. This permit is not transferable. 5. In event the facilities fail to perform satisfactorily including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 6. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 7. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes, and may be rescinded unless the facilities are installed, maintained and operated in a manner which will protect the assigned water quality standards of the surface waters, and prevent any contamination of the ground waters which will render them unsatisfactory for normal use. 8. Solids, sludges, or other pollutants removed or resulting from the wastewater storage facilities shall be contained and disposed of in such a manner as to prevent any contamination of the surface or ground water of the State. 9. Diversion or bypassing of the untreated wastewater from the treatment and disposal system is prohibited. 10. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or ground waters resulting from the operation of this facility. 11. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 12. The Permittee, at least six (6) months prior to the expiration of this Permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if indicated, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 13. An acceptable reserve area, equal in size to the approved disposal area, shall be maintained at all times. 14. The drainfield area after installation of the nitrification lines shall be dressed and seeded to prevent soil erosion. 15. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 16. The Winston-Salem Regional Office, Phone No. (919) 761-2351 shall be notified at least twenty-four (24) hours in advance of backfilling of the installed subsurface disposal system so that an in --place inspection can be made of said system prior to backfilling. Such notification to the Regional Supervisor shall be made during the normal office from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 17. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 18. Grease trap and septic tank shall be equipped with an access manhole having a minimum diameter of 30 inches extending a minimum of six inches above the finished grade. 14. System installation shall follow the contour (drainfield). 20. Soil in the drainfield areas must not be disturbed or removed during the construction phase. 21. The application rate shall not exceed 0.35 gallons per day per square foot. 22. The truck wash shall only be used for the exterior washing of earth moving equipment and trucks. 23. The truck wash shall not be used to wash earth moving equipment and trucks which have entered toxic or hazardous waste areas. 24. The Permittee must maintain a minimum of two (2) feet vertical separation between nitrification lines and the seasonal high water table. Permit issued this the 31st day of October, 1988.
