Dear. Mr. Berry:
In accordance with your application received September 22, 1988, we are
forwarding herewith Permit No. WQ0000488, dated November 10, 1988, to N.C.
Division of Parks and Recreation for the continued operation of the subject spray
irrigation wastewater treatment and disposal facility.
This permit shall be effective from the date of issuance until October 31,
1993, and shall be subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter 150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this
permit shall be final and binding.

For the discharge of Sewage, Industrial wastes, or Other Wastes
In accordance with the provisions of Article 21 of. Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of a 2,350 GPD non -discharge land application spray irrigation
type wastewater collection treatment and disposal facility consisting of
approximately 1,056 linear feet of 8 -inch gravity sewer, 3,370 linear feet of 3 -inch
diameter force main, a 40 GPM pump station with dual pumps and high water alarm, a
210,000 gallon capacity facultative lagoon with 1 HP floating mechanical aerator, a
64,000 gallon capacity storage lagoon, a chlorine contact tank with gas chlorinator,
a 68 GPM irrigation pump station with dual pumps and auto -timer shut-off controls,
approximately 1.6 acres of irrigation area and associated irrigation piping,
sprinklers, and appurtenances to serve the Vista Point Recreation Area at Jordan Lake
with no discharge of wastewater to the surface waters of the State, pursuant to the
application received September 22, 1988, and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the
Department of Natural Resources and Community Development and considered a part of
this permit.
This permit shall be effective from the date of issuance until October 31, 1993,
and shall be subject to the following specified conditions and limitations:
1. Issuance of this permit shall void Permit No. 9562,
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5. The spray irrigation facility shall be effectively maintained and operated at
all times so that there is no discharge to the surface waters, nor any
contamination of ground waters which will render them unsatisfactory for normal
use, In the event that the facilities fail to perform satisfactorily, including
the creation of nuisance conditions or failure of the irrigation area to
adequately absorb the wastewater, the Permittee shall take such immediate
corrective action as may be required by the Division of Environmental Management.
6. The Permittee shall employ a certified wastewater treatment plant operator to be
in responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission.
7. Diversion or bypassing of the untreated wastewater from the treatment is
prohibited.
8. The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner approvable by the North
Carolina Division of. Environmental Management.
4. A suitable year round vegetative cover shall be maintained.
10. Adequate measures shall be taken to prevent wastewater runoff from the spray
field.
11. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and if warranted, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
12. The application rates shall not exceed 0.4 inches/acre/week.
13. Freeboard in the lagoon shall not be less than 2 feet at any time.
14. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
15. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
16. No type of wastewater other than that from the Vista Point Section shall be
sprayed onto the irrigation area.
17. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee
to insure proper operation of the subject facilities.
18. The following minimum buffer distances shall be maintained from the wetted area:
a) 400 feet to nearest existing residence or other place of public assembly.
b) 150 feet to property lines
c) 100 feet to potable water wells or any surface waters or drainage ways.
19. Adequate records shall be maintained by the permittee tracking the amount of
wastewater disposed. These records shall include, but are not necessarily
limited to the following information:
a) date of irrigation,
b) volume irrigated,
c) continuous weekly, monthly, and year-to-date hydraulic loadings (inches/acre).
These records shall be submitted to the Water Quality Supervisor of the Raleigh
Regional Offiice on or before the 10th day of the following month.
20. The two (2) existing monitor wells shall be sampled every March, July and
November for the following parameters:

Total Ammonia
TDS
Water Levels
PH
TOC
Chloride
Total Coliforms
The measurement of water level must be made prior to sampling for the remaining
parameters.
The results of the sampling and analysis shall be sent to the N.C. Division of
Environmental Management on Form GW -59 (Compliance Monitoring Report Form) every
Apra.]., August and December.
21. Any additional groundwater quality monitoring, as deemed necessary by the
Department, shall be provided.
