In accordance with your permit modification request received on March 10, 2011, and subsequent
additional information required for the ownership change for Majestic Oaks received on June 22, 2011,
we are forwarding herewith a modified Attachment A, certified July 1, 2011, to Lewis Farms of Burgaw,
Inc. for the subject residuals land application program.
This certification is being issued to approve additional tonnage for a new residuals source, which
is the Majestic Oaks Wastewater Treatment Plant. The tonnage from this new source is 10 dry tons per
year.
Session Law 2009-406, and Session Law 2010, entitled "An Act to Extend Certain Government
Approvals Affecting the Development of Real Property Within the State," were enacted by the General
Assembly and signed into law. The Acts, known as the Permit Extension Acts of 2009 and 2010, extends
the expiration date of certain government approvals and permits. The subject permit falls within the scope
of these Acts and is therefore being extended to May 31, 2018.
Please replace the existing Attachment A of Permit No. WQ0000455 that was previously
certified on August 20, 2010, with this modified Attachment A. It is the Permittee's responsibility to
ensure that a complete permit is created by inserting this modified Attachment A into Permit No.
WQ0000455, issued on March 16, 2010.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQ0000455 that was previously (most recently) certified on August 20,
2010; and shall be subject to the conditions and limitations as specified therein and well as those specified
in the above -referenced permit. As always, remember to take the time to review this modified
Attachment A thoroughly, as some of the conditions contained therein may have been changed since the
last certification. Note that this modified Attachment A is hereby incorporated by reference into Permit
No. WQ0000455, issued on March 16, 2010.

request within 30 days following receipt of the certification. This request shall be in the form of a written

Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.
