Box 368 Thomasville, NC 27360 SUBJECT: Permit No. 12378 City of Thomasville Alum Sludge Storage & Disposal Facility Phase I Davidson County Dear Mr. Williams: In accordance with your application received May 3, 1985, we are forwarding herewith Permit No. 12378, dated October 25, 1985, to the City of Thomasville for the construction and operation of the subject non -discharge alum sludge storage and disposal facility, This Permit shall be effective from the date of issuance until October 31, 1990, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director within thirty (30) days following receipt of this permit, identifying the specific issues to be con- tended. Unless such demand is made, this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Bolt 27687, Raleigh, North Carolina 27611-7687 Telephone 919-733-7015 An Equal Opportunity Affirmative Action Employer NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the Discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Lags, Rules and Regulations PERMISSION IS HEREBY GRANTED TO City of Thomasville Davidson County FOR THE construction and operation of alum sludge storage and disposal facilities consisting of a 2.1 acre earthen -diked sludge settling lagoon, a receiving station with.a sloped concrete pad and 6 -inch gravity flow pipe to discharge sludge delivered by truck' into, the lagoon, a decanting structure with a height -adjustable telescoping valve and rip -rap effluent discharge channel to discharge supernatant wastewater into an adjacent 4.3 acre disposal area with no discharge of wastewater into the surface waters of the State, pursuant to the application received May 3, 1985 and in conformity with the project plans, specifications, and other supporting data, subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This Permit shall be effective from the date of -issuance until October 31, 1990 and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless ,the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. 5. Solids, sludges, or other pollutants removed or resulting from the wastewater storage facilities shall be contained and disposed of in such a manner as to prevent any contamination of the surface or groundwaters of the State. Permit No. 12378 Page 3 6. The facility shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters. In event the facilities fail to perform satisfactorily, including the creation of nuisance conditions, or failure of the sludge storage lagoon and supernatant disposal area to adequately contain the wastewater, the Permittee shall take such immediate corrective action as may be required by the Division of Environmental Management. 7. Freeboard in the sludge storage lagoon shall not be less than one (1) foot at any time. 8. Adequate diking shall be constructed to prevent surface water run-off from the supernatant disposal area. 9. The earthern dikes and berms shall be adequately stabilized with grass or other suitable year-round cover vegetation. 14. The facilities shall be effectively maintained and operated as a non - discharge system to prevent the discharge of any wastewater into the surface waters of the State. 11. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or ground waters resulting from the operation of this facility. 12. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 13. In any future.transfer of this land, a notice shall be given to the new owner that gives full details as to the materials applied or incorporated at this site. 14. The Permittee, at least six (6) months prior to the expiration of this Permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if indicated, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 15. The Winston-Salem Regional Office, Phone No. 919/761-2351, shall be notified at least twenty-four (24) hours in advance of operation of the installed alum sludge storage system so that an in-place inspection can be made of said facilities prior to operation.
