In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of. North Carolina as amended, and other. applicable Laws, R"Ins, and Regulations
PERMISSION IS HEREBY GRANTED TO

construction and operation of a wastewater recycle system cons:i_sting of a 600 gallon
radiator test vat, a 600 gallon radiator boil out vat_, a 1,200 gallon water proof
septic tank and a 1/3 HP submersible pump to serve Sm i.tlr Rrtd i ntor Service with no
discharge to the surface waters of the State, pursuant to the application received
,Tune.. J. 1988, and in conformity with the project plan., spacifi.cat_ions, and other
supporting data subsequently filed and approved by the D(,par.tment of Natural
Resources and Community Development and considered a part of this permit.
This permit shall be effective from t:he date of issuance unt i 1 August 3.1, 1993,
and shall be subject to the following speci..find conditions tinct 1 imitntinns:
1. This permit shall become voidable unless the fncilktins are constructed irl.
accordance with the approved plans, specifications and ot:hpr srrppouing data.
2. This permit is effective only with respect to the naturn and volume of wastes
described in the application and other supporting data.
3.
The facilities shall be properly
maintained and operated at
all times.
4.
This permit is not transferable.
5.
The solids generated from these treatment: facilities Hurst. be disposed of in
accordance with General Statute
143-215.1 and in a manner

Carolina Division of Environmental. Management.
6.
Satisfactory controls shall be
provided to contain the
accidental
dischntge or
spillage cf hazardous or toxic
substances either stored
or being

project site.

7. Diversion or bypassing of the untreated wastewater from the recycle system is
prohibited.
8. The Permittee, at least six (6) months prior. to Hie. expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, Incl if warranted, will
extend the. Permit for such period of time and rtncier such conditions and
limitations as it may deem appropriate.
9. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwate.r.s resulting from the operation of this
facility.
1.0. The facilities shall, be effectively maintained and operated as a non -discharge
system to prevent the discharge -of any wastewater result:.ing from the operation of
this facility.
11. Adequate inspection, maintenance, and cleaning shall he providod by the Permittee
to insure proper operation of the subject :facilities.
12. Soli -ds, sludges, or other pollutants removed or resnit.ing from the wastewater
storage facilities shall be contained and dispose.(] of in such a manner as to
prevent any contamination of the. surface- or g.roundwat.nrs of the State.
13. A copy of tate approved plans and specifications shall he mI i.ntl:i.ned on file. by
the Permittee for a minimi.rm of five years from thr, date of isscnance.
14. Upon comple-t.i.on of construction. and prior to ohe.ratiou of this permitted
facility, a certification must be received from a professior1n1 engineer
certifying that the permitted facility has been installed i -ii rrccordan.^e with the

1.5. A spare pump shall he kept on premises at all times.
1.6. Operation of facility shall not begin until i roof is ill over thr� i-irw
concrete slab in the work area.
17. Settled soli -ds shall be tested for high lead cont.ont: ultimate disposal.
If the lead content causes this wasl:e to bo. nc n hazlydous waste it:
shall. be disposed of accordingly.
