Wastes, or Other. Wastes In accordance with the provisions of Article 2.1 of Chapter 143, General Statutes of North Carolina as amended, and other_ applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Mr. George D. Blackburn Union County FOR THE construction and operation of a 1.000 GPD non discharge type wastewater treatment anti disposal system consisting of 70 linear feet of 4• -inch gravity sewer, a 1000 galloli grease separation tank, a 1000 gallon settling tank, an 800 gallon pump tank with 10 GPM simplex effluent pump and a high water alarm, 800 lhuQar feet of: 2 -:inch force -main, a 8400 square foot low pressure pipe disposal field with six 2 -inch manifolds with gate. valves, 1.,764 Linear feet of 1 -1%4 -:inch laterals laid :in 28 trenches Ind all associated valves stand pipes and appur.t:e.nu:rans to serve Blackburn Carwas'n located in Union County with no discharge to the surface waters of the State, pursuant to the application received May 25 1988, and in conformity with the project plan, specifications, and other supporting data subsequently filed anti approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the elate of issuance nnti..l. August 31, 1993, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature rind volume of wastes described in the application and other supporting data. 3. This permit is not transferable. 4. Upon completion of construction and prier to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687. 5. A copy of the approved plans and specifications shall be maintained on file by the permittee for a minimum of five years from the date of issuance. 6. The Mooresville Regional Office, phone no. 704-663-1699 shall be notified at least twenty-four (24) hours in advance of operation of the installed system so that an in-place inspection can he made. Such notification t.o the. regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State i{ol.i_days. 7. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be resci.ruled unless the faci.lit.ies ar.e installed, maintained, and operated in a manner which w_i 1.1 protect the assigned water quality standards of the surface waivers and ground waivers. 8. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, incindi.ng the construction of additional or replacement wastewater treatment or disposal facilities. 9. Upon the availability of a municipal or regional sewerage collection system, the subject wastewater treatment facilities shall. be nhandoned and a.l.l wastewater discharged into the municipal or regional sewerage system. 10. The sludge generated from these treatment facilities mist, be disposed or in accordance with General. Statute .1.43-215.1 and in a manner approvable by the North Carolina Division of. Environmental Management.. 11. Diversion or bypassing or the untreated wastewater from Cho Lycatment facilities is prohibited. 1.2. The Permittee, at least six (6) months prior to the expirnt:i.nn of this permit, shall request its extension. Upon. receipt of. Lho request , the Commission will review the adequacy of the facilities de scribed therein, and if warranted, will extend the Permit for such period of time and render such conditions and limitations as it may deem appropriate. 13. The application rages shall not exceed 0.12 GPD/square. foot. 1.4. Adequate facilities shall be provided to prevent surface rnnnff from carrying any disposed or stored material into any surface waters. 15. The facilities shall. be effectively maintained and opornLnd as a non -discharge system to prevent the discharge of any wastewater resulting From the operation of this facility. 16. No type of wastewater other than the carwash water from George Blackburn Carwash shall be disposed off- on the dra.inf_eld. 17. No traffic or equipment shall. be allowed cur Lhe d isposa I niF n excepL while installation occurs or while normal maintenance is being performed. 18. Adequate inspection., maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 1.9. An acceptable reserve area, equal in size to the approved disposal area, shall be mai-ntai.ned at all times. 20. Failure of the Permittee to properly operate this system is subject to a penalty up to $10,000 per day 21. Any additional. groundwater quality monitoring as deemed necessary by the Department, shall be provided. Permit issued this the 11th day of August, 1988.
