SUBJECT: Expiration of Permit
Blanchard, Bill Blanchard SFR
State Permit No. WQ0004343
Alamance County
Jim Johnston of the Winston-Salem Regional Office, spoke with Mr- Blanchard, for the
purpose of determining if Mr. Blanchard wanted to renew his permit. Mr. Blanchard indicated
ZD
that the permitted facility was never constructed. Mr. Blanchard instead built a septic tank -
nitrification field, which he began using August 9, 1991 _ DEM staff have been unable to get Mr
Blanchard to request - in writing - the rescission of this permit. Therefore, the permit is being
allowed to expire, effective December 31, 1995.
Mr. Blanchard is now covered to treat his wastewater under an Alamance County Subsurface
Permit. State Permit No. WQ0004343 will be removed from our computer tracking system,
effective immediately.
