Dear Mr. Patterson:
With regards to your request of May 16, 1991, the Division
is hereby amending permit No. WQ0004332 to reflect the
groundwater monitoring schedule you requested. The monitoring
schedule was inadvertently changed in the permit issued March 28,
1991.
Therefore, condition No. IV.1 is hereby amended as follows:
1. Monitor wells shall be sampled every April,
August, and November.
2. The results of the sampling are due every
May, September, and December.
This amendment does not in any way alter or modify any other
condition or limitation contained in permit No. WQ0004332 issued
March 28, 1991.

Back in September of 1989 we reauested permission to shift cur
monitoring well schedule from March, July, and November to April:
August, and November. Permission was granted and we proceeded
under- the new scheduled frequency. Gur permit has since oeen
renewed and at that time it reverted back to Marsh, July, and
November. We request an amendment to our permit to allow: samples
to be taken in April, August, and November.
Also, our new permit requires monitoring for effluent every March,
July, and November. Our laboratory has requested that the
monitoring months be changed back to April, August, and November
and that future permit renewals specify this schedule.
Enclosed please find a copy of the letter we receivec granting cur
initial request. We apereciate your assistance in this 'Matter.

in accordance with your letter dated, September 26, 1989, we
are amending Permit �+o. 13255 to change the months in which
groundwater quality samples must be collected from the monitor
wells at the subject facility.
Condition No. 25 is hereby modified as follows:
1. Monitor wells shall be sampled every April, August,
and November.
2. The results of the sampling and analysis shall be sent
to the N.C. Division of Environmental Management on
Form GW-59 [Compliance Monitoring Report Form] every
May, September and December.
This permit amendment does not alter any other conditions or
limitations specified in Permit No. 13255. If you have any
