The subject facility requested on May 22, 1991 that their permit not be renewed because the facility
was removed from use. The Winston-Salem Regional Office has confirmed that the permitted facility was
removed from use. Therefore, since the permit has expired and as per the recommendation of the
Winston-Salem Regional Office, the permit is being cleared from the permit tracking system. This
memorandum shall serve as formal "close-out" of the subject files.
By copy of this memo, I am requesting the Budget Office to clear all pending billings, Notices of
Revocation and permit file for the subject facility.
