The revised permit for the addition of one (1) pump station, .its
related force main and resizing certain pipes to acccun-edate. the
additional flea voids Permit No. 1,,Q0003189, dated April 24, 1991
In accordance with your application received August 30, 1991, we
are forwarding herewith revised Permit No. WQ0003189 dated September
24, 1991, to the Town of Chocowinity for the construction and
operation of the subject wastewater collection facilities.
This permit shall be effective from the date of issuance until
rescinded, shall void Permit No. WQ0003189 which was issued on April
24, 1991 and shall be subject to the conditions and limitations as
specified therein.
If any parts, requirements, or limitations contained in this
permit are unacceptable to you, you have the right to an adjudicatory
hearing upon written request within thirty (30) days following receipt
of this permit. This request must be in the form of a written
petition, conforming to Chapter 150E of North Carolina General
Statutes, and filed with the Office of Administrative Hearings, P. O.
Box 11666, Raleigh, N. C. 27604. Unless such demands are made, this
permit shall be final and binding.

One (1) set of final approved plans and specifications is to be
forwarded to you, one (1) set retained for the Environmental Protection
Agency (EPA) .
A copy of the approved plans and specifications shall be maintain-
ed on file by the Owner for the life of the project..
upon completion of construction and prior to operation of this
permitted facility, a certification must be received from a profession-
al engineer certifying that the permitted facility has been installed
in accordance with the approved plans and specifications. nail the
Certification to the Permits and Engineering Unit, P. O. Box 29535,

attached.)

General Statutes of North Carolina as amended, and other applicable
Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

construction and operation of a pressure sewer collection system
consisting of approximately 41,820 linear feet of 1 1./4", 326 linear
feet of 1 1/2", 13,679 linear feet of 2", 9,503 linear feet of 2 1/211,
5,665 linear feet of 3", 15,647 linear feet of 4", 4,307 linear feet of
6" and 29 linear feet of 8" pipe; 367 septic tanks with pumps,
including controls; a gravity portion of the system includes 10,875
linear feet of 4", 1,595 linear feet of 6" pipe; the STEP system shall
also include six (6) - 1,000 gallon septic tanks, ten (10) - 2,000
gallon septic tanks, and seven (7) pump stations with duplex pumps
rated at 10, 80, 37, 40, 10, 15, 90 GPM; and the 350 GPM duplex Main
pump station with approximately 17,624 linear feet of 8" force main,
of which approximately 1,300 linear feet of force main to be installed
inside a carrier pipe is for river crossing and 100 MI standby

generator, to serve the Town of Chocowinity and the discharge of
107,336 GPD of collected residential wastewater into the City of
Washington's existing sewerage system,
pursuant to the application received August 30, 1991 and in conformity
with the project plan, specifications, and other supporting data
subsequently filed and approved by the Department of Environment,
Health and Natural Resources and considered a part of this permit.
This Permit modification shall be effective from the date of
issuance until rescinded, shall void permit No. WQ0003189, which was
issued on April 24, 1991 and shall be subject to the following
conditions and limitations:
1. This Permit shall become voidable unless the facilities are
constructed in accordance with the approved plans, speci-
fications and other supporting data.
2. This Permit is effective only with respect to the nature and
volume of wastes described in the application and other
supporting data.
3. The facilities shall be properly maintained and operated at
all times.
4. The sewage and wastewater collected by this system shall be
adequately treated in the City of Washington's Wastewater
Treatment Plant prior to being discharged into a receiving
stream.
5. Diversion or bypassing of the untreated wastewater from the
treatment facilities is prohibited.
6. The facilities shall be constructed in accordance with the
requirements of the North Carolina Sedimentation Pollution
Control Act.
7. Construction of the sewers, pump station(s) and force main
shall be scheduled so as not to interrupt service by the
existing utilities nor result in an overflow or bypass
discharge of wastewater to the surface waters of the State.
8. Failure to abide by the conditions and limitations contained
in this permit may subject the Permittee to an enforcement
action by the Division of Environmental Management in
accordance with North Carolina General Statute 143-215.6.
9. The issuance of this permit does not preclude the Permittee
from complying with any and all statutes, rules, regulations,
or ordinances which may be imposed by other government
agencies (local, state, and .federal) which have jurisdiction.
10. The Permittee shall provide for the pump station and force
main the following items:
a. Pump on/off elevations located so that 2--8 pumping
cycles may be achieved per hour in the pump station.
b. An air relief valve located at all high points along the
force main.
C. A screened vent for the wet well.
d. Fillets located in the wet well at the intersection of
the flooring and sidewalls.
e. Three feet of cover (minim=) over the force main or the
use of ferrous material where three feet cannot be
maintained.
f. Sufficient devices which will protect the pump station
from vandals.
g. Flood protection if the pump station is located below
the 100-year flood elevation.
11. In the event that the facilities fail to perform satisfactor-
ily, including the creation of nuisance conditions, the
Permittee shall take immediate corrective action, including
those as may be required by this Division, such as the con-
struction of additional or replacement wastewater collection
facilities.
12. NONCOMPLIANCE NOTIFICATION:
The Permittee shall report by telephone to the Washington
Regional Office, telephone No. (919)946--6481 as soon as
possible, but in no case more than 24 hours or on the next
working day following the occurrence or first knowledge of
the occurrence of either of the following:
a. Any process unit failure, due to known or unknown
reasons, that render the facility incapable of adequate
wastewater transport such as mechanical or electrical
failures of pumps, line blockage or breakage, etc.
b. Any failure of a primping station or suer line resulting
in a by-pass directly to receiving waters without
treatment of all or any portion of the influent to such
station or facility.
Persons reporting such occurrences by telephone shall also file a
written report in letter ford within 15 days following first
knowledge of the occurrence. This report must outline the actions
taken or proposed to be taken to ensure that the problem does not
recur.

13. This permit shall become voidable unless the agreement between the
Town of Chocowinity and the City of Washington for the collection
and final treatment of wastewater is in full force and effect.
