Director SUBJECT: Permit No. WQ0003189 Town of Chocowinity Pressure Sewer Collection & Transportation System Project No. C370797-01 Beaufort County In accordance with your application received March 5, 1990, we are forwarding herewith Permit No. WQ0003189 dated April 24, 1991, to the Town of Chocowuuty for the construction and operation of the subject wastewater collection facilities. This permit shall be effective from the date of issuance until rescinded, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 1508 of North Carolina General Statutes, and filed with the Office of Administrative Hearings, P. O. Box 11666, Raleigh, N. C. 27604. Unless such demands are: made, this permit shall be final and binding. One (1) set of final approved plans and specifications is to be forwarded to you, one (1) set retained for the Environmental Protection Agency (EPA). Mail the Certification to the Permits and Engineering Unit, P. O. Box 29535, Raleigh, North Carolina 27611-7687. (Engineer's Certification Form is attached.) Coney, Jr., Secretary Director NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMISSION DEPAR"IM NT OF ENVIRONMENT, HEALTH, AND NATURAL RESOURCES RALEIGH PERMIT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Hiles, and Regulations PERMISSION IS HEREBY GRANTED TO Town of Chocowinity Beaufort County FOR THE construction and operation of approximately 41,820 linear feet of 1 1/411, 326 linear feet of 1 1/2", 13,679 linear feet of 2", 10,731 linear feet of 2 1/2", 6,987 linear feet of 3", 21,385 linear feet of 4", 4,112 linear feet of 6" and 29 linear feet of 8" pipe. The STEP system- shahaso-include six-(5) - 1;000-gallon septic tanks, n (9) - 2,000 gallon septic tanks, 367 septic tanks with pumps, including controls and six (6) pump stations with duplex pumps rated at 10, 80, 37, 40, 10 and 15 GPM; and a 350 GPM duplex main pump station with approximately 17,624 linear feet of 8" force main, of which approximately 1,300 linear feet is for river crossing pipe, to serve the Town of Ch000winity and the discharge of 107,336 GPD of collected residential wastewater into the City of Washington's existing sewerage system, PoDution Prevention Pays P_O. This Permit shall be effective from the date of issuance until rescinded, and shall be subject to the following conditions and limitations: 1. This Permit shall become voidable unless the facilities are constructed in accordance with the approved plans, speci- fications and other supporting data. 2. This Permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. The sewage and wastewater collected by this system shall be adequately treated in the City of Washington's Wastewater Treatment Plant prior to being discharged into a receiving stream. 5. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 6. The facilities shall be constructed in accordance with the requirements of the North Carolina Sedimentation Pollution Control Act. 7. Construction of the sewers, pump station(s) and force main shall be scheduled so as not to interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the State. 8. Failure to abide by the conditions and limitations contained in this permit may subject the Perrmittee to an enforcement action by the Division of Environmental Management in accordance with North Carolina General Statute 143--215.6. 9. The issuance of this permit does not preclude the Permittee from complying with anv and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. - 3 - 10. The Permittee shall provide for the pump station and force main the following items: a. Pump on/off elevations located so that 2-8 pumping cycles may be achieved per hour in the pump station. b. An air relief valve located at all high points along the force main. C. A screened vent for the wet well. d. Fillets located in the wet well at the intersection of the flooring and sidewalls. e . Three feet of cover (nd n i m„r„) over the force ma n or the use of ferrous material where three feet cannot be maintained. f. Sufficient devices which will protect the pump station from vandals. g. Flood protection if the pump station is located below the 100-year flood elevation. 11. In the event that the facilities fail to perform satisfactor- ily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the con- struction of additional or replacement wastewater collection facilities. 12. NONCOMPLIANCE NOTIFICATION: The Permittee shall report by telephone to the Washington Regional Office, telephone No. (919)946-6481 as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of either of the following: a. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater transport such as mechanical or electrical failures of pumps, line blockage or breakage, etc. b. Any failure of a pumping station or sewer lane resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. Persons reporting such occurrences by telephone shall also file a written report in letter form within 15 days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. - 4 - 13. This pe udt shall become voidable unless the agreement between the Town of Chocowinity and the City of Washington for the collection and final treatmant of wastewater is in full force and effect.
