A. SDR 11-13 Annlication:
1. Applicant's Certification on Page 3 of 3 on Form SDR 11-13 is signed by William S. Bourke. A
delegation letter from the Town Mayor or Town Manager needs to be submitted which authorizes
William S. Bourke to sign the application on behalf of the Town in accordance with 15A NCAC
02T .0106(c). Alternatively, the application may be signed by either the Town Mayor or Town
Manager in accordance with 15A NCAC 02T .0106(b)(4).
Please submit a revised Form SDR 11-13 signed by either the Town Mayor or Town Manager, or
submit a delegation letter from the Town Mayor or Town Manager giving William S. Bourke
authority to sign on behalf of the Town.
