POST OFFICE BOX 126 SPRING HOPE, NORTH CAROLINA 27885 Subj ect: Attachment A Certification Permit No. WQ0002004 Bass Farms, Inc, — Bass Farms Sausage Wastewater Irrigation System Nash County Dear Mr. Bass: It has come to our attention that the effluent sampling required under the subject permit is in need of revision. To address this situation, we are forwarding herewith a modified Attachment A, certified December 3, 2010, to Bass Farms, Inc. for the subject wastewater surface irrigation system. On August 5, 2009, Session Law 2009-406, entitled "An Act to Extend Certain Government Approvals Affecting the Development of Real Property Within the State," was enacted by the General Assembly and signed into law. The Act, known as the Permit Extension Act of 2009, extends the expiration date of certain government approvals and permits. On August 2, 2010, this Act was extended an additional year. The subject permit falls within the scope of this Act and is therefore being extended to October 31, 2013. This certification is being issued to standardize the sampling schedule to three times a year (March, July, and October), and to remove Fecal Coliform (Parameter Code 31616) and Total Nitrate Nitrogen (Parameter Code 00620). Please replace the existing Attachment A of Permit No. WQ0002004 that was previously certified on June 9, 2010, with this modified Attachment A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A into Permit No. WQ0002004, issued on June 9, 2010 This modified Attachment A shall be effective from the date of certification; shall void Attachment A of Permit No. WQ0002004 that was previously (most recently) certified on June 9, 2010; and shall be subject to the conditions and limitations as specified therein and well as those specified in the above -referenced permit. As always, remember to take the time to review this modified Attachment A thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment A is hereby incorporated by reference into Permit No. WQ0002004, issued on June 9, 2010. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
