In accordance with your permit modification request received January 30, 2018, and subsequent
additional information received March 18, 2018, March 22, 2018, and March 26, 2018, we are forwarding
herewith a modified Attachment B certified April 2, 2018, to the City of Mount Holly for the subject
residuals program.
This certification is being issued to modify Attachment B by
1. Addition of three fields in Cleveland County totaling 89.43 acres, those fields being NC -CL -
11 -01, NC -CL -11-02 and NC -Cl -12-01.
2. Addition of two fields in Lincoln County totaling 30.66 acres, those fields being NC -LT -44-01
and NC -LT -45-01.
3. Removal of four fields in Lincoln County totaling 46.6 acres, those fields being NC -LT -43-01
to -04.
In the past, field NC -LT -44-01 has received both turkey and chicken litter on the field. It is
imperative to stress that all nutrient sources shall be accounted for in the annual report, not only for this
field but for all fields.
This modified Attachment B shall be effective from the date of certification until November 30,
2019 (i.e., expiration date of the current permit), shall void Attachment B of Permit No. W00001863 issued
December 1, 2014, and shall be subject to the conditions and limitations as specified therein, as well as
those specified in the above -referenced permit. Please replace the Attachment B of Permit No. WQ0001863
that was issued December 1, 2014 with this modified Attachment B.
Please note that the maps for both the land application sites in the December 1, 2014 permit and
the new land application sites are attached to this letter. In addition, the setback waivers contained in the
December 1, 2014 are contained in this permit attachment revision. Any old maps should be removed from
the December 1, 2014 permit and replaced with the maps provided.

If any parts, requirements, and/or limitations contained in this certification or modified Attachment
B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30
days following receipt of the certification. This request shall be in the form of a written petition, conforming
to Chapter 150B ofthe North Carolina General Statutes and filed with the Office of Administrative Hearings
at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this
permit shall be final and binding.

information requested. Failure to submit all of the required items will lead to additional processing and review time for the
permit application. Unless otherwise noted. the Applicant shall submit one orieinal and two copies of the application and
supporting documentation

General — This document is an agreement between the Applicant and the owner(s) of the property adjacent to the land application
site(s) to allow setback reduction for habitable residences or place of public assembly not owned by the permittee or not to be
maintained as part of project site(s) in accordance with 15A'_NCAC 02T .1108tb1(2)jfor habitable residences or place of public
assembly only). Do not submit this agreement for review without a corresponding application form (FORM: RLAP I1-13).
A. Setback Waiver Agreement:
❑ Submit the completed and appropriately executed Setback Waiver Agreement (FORM: SWA 11-13) form. Please do not
make any unauthorized content changes to this form. If necessary for clarity or due to space restrictions, attachments to the
application may be made, as long as the attachments are numbered to correspond to the section and item to which they refer.
❑ Prepare a separate agreement for each property from which a waiver is being sought.
❑ Any other setbacks required by 15A NCAC 02T .1108, other than those referenced above, cannot be waived through
execution of this agreement.
❑ A copy of the completed and executed agreement must be provided to the property owner.
