In accordance with your permit modification request received June 4, 2015, we are forwarding
herewith modified Attachment A, certified August 14, 2015 to City of Lexington for the subject residuals
program.
This certification is being issued to modify Attachment A by: Addition of 2 tons of residuals from
KAO of High Point, NC.
This modified Attachment A shall be effective from the date of certification until December 31,
2015 on a one-time experimental basis. Thereafter, the previous Attachment A shall be effective. Please
replace the Attachment A of Permit No. WQ0001318 issued on May 1, 2014, with this modified
Attachment A.
Please note that only the maps for any new residuals -generating sources and/or land application
sites are attached to this letter. It is the Permittee's responsibility to ensure that a complete permit is
created by inserting these documents into the previously issued permit.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.

1. Maximum Dry Tons per Year for residuals source is the amount of residuals approved for composting. Maximum Dry Tons per Year for Class A residuals is the amount of residuals product
approved for distribution from the permitted facility.
2. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under permit Condition 1V.2,
3. Testing of metals and nutrients as stipulated under permit Condition N.3.
4. Analyses of pathogen and vector attraction reductions as stipulated under permit Condition IVA.
5. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above.

6. The amount is not limited; however, the Permittee shall monitor and keep records of the amount used for composting each year.
7. On an experimental basis up to 2 tons of materials from KAO specialty chemical company of High Point NC, may be amended to the residuals of the City of Lexington's composted waste. This
shall occur in the time frame of August 14, 2015 to December 31, 2015. If the City of Lexington desires to make addition of this material to its residuals, the City of Lexington
shall apply for a permit modification supplying the results of this experiment as part of the amendment process.
