In accordance with your permit modification request received May 22, 2018, and subsequent
additional information received August 7, 2018 and August 28, 2018, we are forwarding herewith a
modified Attachment B certified September 10, 2018, to the City of Burlington for the subject residuals
program.
This certification is being issued to modify Attachment B by:
Adjustments in acreage to fields with the removal of previously required 50 -foot property
buffers between contiguously -owned properties affecting the following fields: NC -AM -009-
01 to -03, NC -AM -013-01 to -03, NC -AM -014-01 to -05, NC—AM-015-01 to -04; NC -AM -

10; NC -AM -119-01 and -2; NC -AM -120-01; NC -AM -121-01; NC -AM -123-01; NC -AM -125-

2. Removal of four fields totaling 46.7 acres with two fields in Alamance County (20.7 acres, NC -
AM -118-06 and NC -AM -118-10) and two fields in Chatham County (26.0 acres, NC -CH -095-
01 and -02).

This modified Attachment B shall be effective from the date of certification until July 31, 2022
(i.e., expiration date of the current permit), shall void Attachment B of Permit No. WQ0000520 issued
August 1, 2017, that was previously certified on November 28, 2017, and shall be subject to the conditions
and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace
the Attachment B of Permit No. WQ0000520 that was certified on November 28, 2017 with this modified
Attachment B.
Please note that only the maps for modified land application sites are attached to this letter and
should be substituted for the previous maps with the same numbers. It is the Permittee's responsibility to
ensure that a complete permit is created by inserting these documents into the previously issued permit.
If any parts, requirements, and/or limitations contained in this certification or modified Attachment
B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30
days following receipt ofthe certification. This request shall be in the form of a written petition, conforming
to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings
at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this
permit shall be final and binding.
