They are both linked to the same company information. I will need to make a change so that doesn't happen again or you can designate an owner affiliation contact that could represent the two facilities. Attached is the letter that we sent out (instructions on how to obtain your electronic COC) and you'll see it shows Lance Davis instead of Stephen LePage — who you had requested. I have an appointment early afternoon but will update the other changes you mentioned in the email. Please let me know if you have any other questions. I submitted an updated "Permit Owner Affiliation Designation Form" in March of this year. At the time, I did confirm that the information was updated, but I checked today and it has since been updated with incorrect information. I believe the changes were triggered when we initiated the opening of our facility in Wilmington NC. I say this because the new owner affiliation is the plant manager of that facility. I have attached the form for your review and approval. Also, the changes I submitted to Laura have not been fully updated. I have also attached that form indicating the correct information. I never received anything on the renewal status of our permit. The last info I received was the request to mark the facilities outfall locations. This was completed and I thought that the permit would be renewed. How do I go about checking on the status of the permit? It sounds like you are working on getting that annual fee straightened out, so the next thing we need to do is get the contacts for this permit and facility squared away. Attached is a report of what we currently have in our database. First, I see that we need an Owner Affiliation designation (a person) — that is the person legally responsible for the permit. That's important for our letters to get to the right people. Right now, no one is listed. To update it, please use this on-line form below. There will be a signed form that needs to be uploaded with it (we need a signature because it's the person legally responsible) -- you will find a link to that in the steps of the form: https://edocs.deg.nc.gov/Forms/SW-Owner-AffiIiation To revise the other contacts (facility, permit, etc.) just respond via email to Laura Alexander (copied above).
