Page#1 DocuSign Envelope ID: C36D71F6-1AF5-469E-8140-3943F1F09BBF 8/30/2018 8/30/2018 NPDES yr/mo/day 18/08/29 Inspection Type C3111218 1 Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) I (Daniel Boss) conducted a Compliance Evaluation Inspection at the Dana Hill WWTP. I met with Danielle Hunter (ORC) and Juanita James (Backup ORC). Overall the plant appeared to be operating okay, but there were a few issues that should be addressed. The blower guards were not covering two of the running blowers. Ms. Hunter said that the blowers overheat when the covers are on them. Not having the guards covering the blowers is a safety hazard and needs to be corrected. The composite sampling needs to be flow proportional rather than time-based. One of the secondary clarifiers had a lot of pin floc and floating sludge. Ms. Hunter said that the skimmer for that particular clarifier can't be run for long without it causing solids to rise in the clarifier. The effluent flow meter was having issues with the LCD display. It was flashing through different screens and appeared to have a few glitches on the readout. There was no documentation for the last calibration of the flow meter. NC0073393 17 (Cont.) Is the DO level acceptable?(1.0 to 3.0 mg/l) One of the aeration basins had about 30% foam covering the surface. The blowers for the aeration basins did not have the guards covering the belts and pulleys because it causes the blowers to overheat. The uncovered belts and pulleys are a safety hazard and need to be addressed. The basin DO values are typically 0.6, 1.6, and 4.0 mg/L according to Ms. Hunter. Is the overflow clear of excessive solids/pin floc? Is the sludge blanket level acceptable? (Approximately ¼ of the sidewall depth) The blankets were 1.5 ft or less in all 3 clarifiers. One clarifier did have a substantial amount of pin floc in it and lots of floating solids. Ms. Hunter said that was because she could only run that particular clarifier's skimmer for the time she was on site. Otherwise it would cause solids to rise in the clarifier. Is sample collected below all treatment units? Is proper volume collected? Is the tubing clean? # Is proper temperature set for sample storage (kept at less than or equal to 6.0 degrees Celsius)? Is the facility sampling performed as required by the permit (frequency, sampling type representative)? Composite sampling is currently time based, but must be flow proportional. The tubing did not appear to be clean. Mrs. James says it is difficult to change the tubing for this sampler because the line is run underground. Comment: Flow Measurement - Effluent Yes No NA NE # Is flow meter used for reporting? Is flow meter calibrated annually? Is the flow meter operational? (If units are separated) Does the chart recorder match the flow meter? Mrs. James said that the meter had been calibrated last year, but there was no documentation of that calibration event at the plant. The meter readout was having issues during the inspection. The LCD display was flashing through different screens and seemed to be displaying glitches. Comment: Effluent Pipe Yes No NA NE Is right of way to the outfall properly maintained? Are the receiving water free of foam other than trace amounts and other debris? If effluent (diffuser pipes are required) are they operating properly?
