Permit: WQ0021861 Owner - Facility: Grady K Ackerman
Inspection Date: 05/30/2018 Inspection Type : Compliance Evaluation Reason for Visit: Routine
Inspection Summary:
This single-family system serves two homes. There is a separate septic tank at the daughter's home, with flow plumbed to
the system via the owner's septic tank. Owner documents weekly and monthly checks and notes system maintenance
(contracted with Sid Gaskins). High water alarms were functional, chlorination appeared functional, but the permit does not
require sampling. The following maintenance items were discussed while on-site:
Replacement of cracked lids on both septic tanks and riser on owner's tank.
Pumping of solids from both septic tanks and both peat filters, upon receipt of replacement
peat material.
The liner in the wet weather lagoon needs repair for several tears and removal of woody
plants. The owner says there is almost never any liquid in the lagoon (only an inch with the
recent heavy rains for a short period). It is difficult to judge with a recent base level
liner tear if any water wouldn't just drain; however, MRO staff have not observed water in
this lagoon during past inspections. The lagoon has always appeared to be an over -build for
this system and the unit a frustration for the owner. MRO recommended he discuss with
a design engineer for input to CO staff for proposed removal; And, weigh against cost of
continued maintenance. A permit modification would be necessary to remove the lagoon.
MRO staff should call in six months for status of repairs.

Are Treatment facilities consistent with those outlined in the current permit? 0 ❑ ❑ ❑
Do all treatment units appear to be operational? (if no, note below.) 0 ❑ ❑ ❑
Comment: The lid and riser on Mr. Ackerman's septic tank was recently damaged with his lawn mower and
needs to be replaced. A lid on the second septic tank at his daughter's home was cracked and
also needs replacement. See notes on peat filter below.
Treatment Filters Yes No NA NE
Is the filter media present? ❑ ❑ ❑

Is the clear well free of excessive solids? ❑ ❑ ❑
Is the mud well free of excessive solids and filter media? ❑ ❑ ❑
Does backwashing frequency appear adequate? ❑ ❑ 0 ❑
Comment: This is a peat filter system. The peat is still functioning, but due for replacement as evidenced by
compaction with ponding at surface in both units. A septage hauler will be scheduled after fresh
peat is delivered. Both septic tanks and peat units will be pumped, with the new peat replaced
the same day for continuous operation. Per owner, septic tanks were pumped about 4 years
ago, but peat is original material.
Treatment Sludge Storage/Treatment
Is the aeration operational?
Yes No NA NE
❑❑■❑
Is the aeration pattern even? ❑ ❑ M ❑
If required, are Sanitary "Ts" present in tankage? ❑ ❑ 0 ❑
Comment: This is a SFR Non -discharge system. There are two Septic tanks (the owner home and their
daughters home), both of which are due for cleanout of scum layer and solids build-up.
Treatment Disinfection Yes No NA NE

❑ ❑
If yes, then what is the EPA twelve digit ID Number? (1000 )
If yes, then when was the RMP last updated?
Comment: This is a single-family system with one tablet chlorine contact chamber. There was no evidence
of solids on the tablet chlorinator when removed for inspection,
but visiblity into the contact well
is too limited to assess. The system design does not allow for
good examination of the effluent
after peat filtration or at the chlorine chamber.

Comment: Both homes are supplied by on-site wells within 250ft of their respective
septic tanks. Recent
heavy rains created soggy areas down -gradient of the drip fields.
It was not raining at the time of
inspection, so the pump was turned on for a short time to assess the driplines in both zones.
There was no evidence of a line break, at the time of inspection.
The lagoon liner needs repair.
