Location description: The proposed proiects involves the installation of a new 30 -inch waterline originating off near the
proposed Access Path C off Lowery Street, headed south over the proposed future greenwav corridor, where it turns west and
runs through an existing double arch culvert under the railroad track while running parallel to Brushy Fork Creek, turns
south to cross Brushy Fork Creek and runs parallel along Brushy Fork Creek, crosses under Bulkmatic Driveway and
Reynolds Park Road, crosses the Duke Energy Transmission line corridor, crosses parallel to Salem Creek to tie into an
existing 30 -inch water main, south of Martin Luther King Jr. Drive, in Winston Salem, North Carolina.
Description of projects area and activity: This verification authorizes the excavation of three crossings of Brushy Fork Creek,
temporarily impacting a total of 124 linear feet of Brushy Fork (Stream 4 — 40 linear feet, Stream 7 — 69 linear feet, and
Stream 8 —15 linear feet) associated with the construction and installation of a new 30 -inch water main. Construction of the
utility line would also temporarily impact 40 linear feet of Salem Creek (Stream 2), along with two unnamed tributaries to
Salem Creek (Stream 1 —19 linear feet and Stream 3 —15 linear feet). In addition, the proiect will impact 246 linear feet of
stream channel associated with the streambank stabilization along Brushy Fork. Finally, there will be a discharge of no more
than 25 cubic yards below the plane of ordinary high water of Brushy Fork associated with protection of the new water main
and proposed greenwav, permanently impacting a total of 75 linear feet of stream.

Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the enclosed
Conditions, your application signed and dated 4/19/2018, and the enclosed plans dated March 2018. Any violation of the attached
conditions or deviation from your submitted plans may subject the permittee to a stop work order, a restoration order, a Class
I administrative penalty, and/or appropriate legal action.
This verification will remain valid until the expiration date identified below unless the nationwide authorization is modified, suspended
or revoked. If, prior to the expiration date identified below, the nationwide permit authorization is reissued and/or modified, this
verification will remain valid until the expiration date identified below, provided it complies with all requirements of the modified
nationwide permit. If the nationwide permit authorization expires or is suspended, revoked, or is modified, such that the activity would
no longer comply with the terms and conditions of the nationwide permit, activities which have commenced (i.e., are under construction)
or are under contract to commence in reliance upon the nationwide permit, will remain authorized provided the activity is completed
within twelve months of the date of the nationwide permit's expiration, modification or revocation, unless discretionary authority has
been exercised on a case-by-case basis to modify, suspend or revoke the authorization.
Activities subject to Section 404 (as indicated above) may also require an individual Section 401 Water Quality Certification. You
should contact the NC Division of Water Resources (telephone 919-807-6300) to determine Section 401 requirements.
For activities occurring within the twenty coastal counties subject to regulation under the Coastal Area Management Act (LAMA), prior
to beginning work you must contact the N.C. Division of Coastal Management Morehead City, NC, at (252) 808-2808.
This Department of the Army verification does not relieve the permittee of the responsibility to obtain any other required Federal, State
or local approvals/permits.
