1. CONTRACTOR MUST SUBMIT ALL PAPER WORK
AND CERTIFICATION FOR ALL SEED MIXES TO
ENGINEER FOR APPROVAL PRIOR TO
PURCHASING AND INSTALLATION.
2. SEED MIX SHALL BE CERTIFIED THAT THE PURE
LIVE SEED (PLS) PERCENTAGE IS EQUAL TO OR
GREATER THAN THAT WHICH IS SPECIFIED ON
THE PLANT SCHEDULES. IF THE PLS IS LESS
THAN SPECIFIED, THE CONTRACTOR SHALL
INCREASE THE SEEDING RATE TO COMPENSATE
FOR THE PLS DIFFERENCE AT HIS/HER OWN
EXPENSE.
3. STRAW MULCH - STRAW MULCH SHALL
CONSIST OF UN -ROTTEN SMALL GRAIN STRAW.
ALL MULCH SHALL BE FREE FROM STATE AND
FEDERAL PROHIBITED NOXIOUS WEED SEEDS
LISTED AS ALIEN INVASIVE SPECIES BY THE
VIRGINIA DEPARTMENT OF CONSERVATION AND
RECREATION NATURAL HERITAGE PROGRAM,
THE US DEPARTMENT OF AGRICULTURE AND
THE MID -ATLANTIC EXOTIC PEST PLANT
COUNCIL.
4. ALL AREAS DISTURBED BY CONSTRUCTION
SHALL BE SEEDED IN ACCORDANCE WITH THE
SEEDING SCHEDULE.
5. NO SEEDING SHALL BE PERFORMED ON FROZEN
GROUND OR WHEN THE TEMPERATURE IS 32F
/ OC OR LOWER.

INSECT PEST EGGS. AND ALL FORMS OF INFESTATION.
ALL PLANT MATERIAL SHALL BE FRESH, FREE FROM
TRANSPLANT SHOCK OR MSIBLE WILT. PLANTS DEEMED
UNHEALTHY SHALL BE REJECTED.
4. ALL PLANT MATERIAL SHALL MEET THE MINIMUM
SPECIFICATIONS AND STANDARDS DESCRIBED IN THE
CURRENT ISSUE OF "THE AMERICAN STANDARD FOR
NURSERY STOCK", PUBLISHED BY THE AMERICAN
ASSOCIATION OF NURSERYMEN, 1250 1 STREET, N.W.,
SUITE 500, WASHINGTON, D.C. 20005.
5. ALL CONTAINER STOCK SHALL HAVE BEEN PROPAGATED
IN A CONTAINER LONG ENOUGH FOR THE ROOT SYSTEM
TO HAVE DEVELOPED SUFFICIENTLY TO HOLD ITS SOIL.
CONTAINER STOCK NTH POORLY DEVELOPED ROOT
SYSTEMS SHALL NOT BE ACCEPTED.
6. PLANTS SHALL BE PREPARED FOR SHIPMENT IN A
MANNER THAT SHALL NOT CAUSE DAMAGE TO THE BARK,
BUDS, BRANCHES, STEMS, OR OVERALL SHAPE OF THE
STOCK. CONTAINER GROWN PLANTS SHALL BE
TRANSPORTED IN THE CONTAINERS IN MICH THEY HAVE
BEEN GROWN.
7. PLANTS NOT INSTALLED ON THE DAY OF ARRIVAL AT
THE SITE SHALL BE STORED AND PROTECTED BY THE
CONTRACTOR. OUTSIDE STORAGE AREAS SHALL BE
SHADED AND PROTECTED FROM THE WIND AND SUN.
PLANTS STORED ON SITE SHALL BE PROTECTED FROM
ANY DRYING AT ALL TIMES BY COVERING THE BALLS OR
ROOTS WITH MOIST SAWDUST, WET BURLAP, WOOD CHIPS,
SHREDDED BARK, PEAT MOSS, OR OTHER SIMILAR
MULCHING MATERIAL.
8. NO SUBSTITUTIONS IN SIZE OR VARIETY OF PLANT
MATERIAL SHALL OCCUR WITHOUT THE PRIOR APPROVAL
OF THE ENGINEER OR LANDSCAPE ARCHITECT.
9. THE CONTRACTOR SHALL VERIFY ALL DIMENSIONS IN THE

PLANTING SEQUENCE
1. HOLES FOR INDIVIDUAL PLANTINGS SHALL BE EXCAVATED
TO PRODUCE VERTICAL SIDES AND FLAT BOTTOMS. ALL
PLANTING HOLES SHALL HAVE ROUGHED, SCARIFIED
SIDES AND BOTTOMS.
2. THE CONTRACTOR SHALL APPLY AGRIFORM FOREST
STARTER TABLETS OR EQUIVALENT PRODUCT TO EACH
PLANT AS PER MANUFACTURER'S DIRECTIONS ON LABEL
AT TIME OF PLANTING.
3. CONTAINERIZED PLANTS SHALL BE SET IN THE PLANTING
PIT AT THE PROPER DEPTH ON TAMPED SOIL. SOIL
REMOVED FROM THE PLANTING PIT SHALL THEN BE
FILLED AROUND THE ROOTS AND TAMPED.
4. THE CONTRACTOR SHALL RESTORE DISTURBED AREAS TO
INDICATED FINAL GRADES IF DISTURBED BY THE
INSTALLATION OF SHRUBS AND TREES.
5. THE PLANTING AREA SHALL BE COVERED NTH 3" - 4"
OF SHREDDED HARDWOOD MULCH FOLLOWING
INSTALLATION OF PLANT MATERIAL. THE CONTRACTOR
SHALL EDGE THE PLANTING BED.
PROJECT MAINTENANCE AND WARRANTY
1. CONTRACTOR SHALL MAINTAIN INSTALLED WOODY
PLANTS AND SEEDED AREAS WITHIN THE PROJECT LIMITS
UNTIL FINAL ACCEPTANCE OF THE PROJECT.
2. THE CONTRACTOR SHALL GUARANTEE 100% SURMVAL
FOR ALL PLANT MATERIAL AFTER THE FIRST COMPLETE
GROWING SEASON.
3. THE CONTRACTOR IS RESPONSIBLE FOR REMOVING ALL
STAKES AND GUY WIRES USED TO SUPPORT TREES
AFTER A PERIOD OF ONE YEAR.

STREAM CHANNELS SHALL BE RESTRICTED TO THE MINIMUM WIDTH
PRACTICABLE AND SHALL NOT EXCEED 30' WIDE FOR GAS UTILITY LINES
AND 20' WIDE FOR ALL OTHER UTILITY LINES EXCEPT AT MANHOLE
LOCATIONS. 15' X 15' PERPENDICULAR VEHICLE TURN–AROUNDS ARE
ALLOWED IN ACCESS CORRIDORS BUT MUST BE SPACED AT LEAST 500'
APART. EXCEPTIONS TO THIS CONDITION REQUIRE WRITTEN APPROVAL FROM
THE PROJECT ENGINEER.
3. THE CONTRACTOR SHALL HAVE A SPECIFIC PLAN FOR RESTORING WETLAND
CONTOURS. ANY EXCESS MATERIAL SHALL BE REMOVED TO A HIGH GROUND
DISPOSAL AREA. THE MIXING OF TOPSOIL AND SUBSOILS WITHIN WETLANDS
ALONG UTILITY CORRIDORS SHALL BE MINIMIZED TO THE GREATEST EXTENT
PRACTICABLE DURING EXCAVATION. THE SOILS SHALL BE PLACED ON FABRIC
TO MINIMIZE IMPACTS WHENEVER POSSIBLE. TOPSOIL EXCAVATED FROM
UTILITY TRENCHES SHALL BE PILED SEPARATELY FROM SUBSOILS AND
BACKFILLED INTO THE TRENCH ONLY AFTER THE SUBSOILS HAVE BEEN
PLACED AND COMPACTED.
DOT CONSTRUCTION AND MAINTENANCE ACTIVITIES MANUAL, SUCH AS
SANDBAGS, ROCK BERMS, COFFERDAMS, AND OTHER DIVERSION STRUCTURES
SHALL BE USED TO MINIMIZE EXCAVATION IN FLOWING WATER. EXCEPTIONS
TO THIS CONDITION REQUIRE WRITTEN APPROVAL FROM THE PROJECT
ENGINEER.
CULVERTS SHALL BE INSTALLED IN SUCH A MANNER THAT THE ORIGINAL
STREAM PROFILES ARE NOT ALTERED AND ALLOW FOR AQUATIC LIFE
MOVEMENT DURING LOW FLOWS. THE DIMENSION, PATTERN, AND PROFILE OF
THE STREAM ABOVE AND BELOW A PIPE OR CULVERT SHALL NOT BE
MODIFIED BY WIDENING THE STREAM CHANNEL OR BY REDUCING THE DEPTH
OF THE STREAM IN CONNECTION WITH THE CONSTRUCTION ACTIVITY.
11. APPLICATION OF FERTILIZER TO ESTABLISH PLANTED/SEEDED VEGETATION
WITHIN DISTURBED RIPARIAN AREAS SHALL COMPLY WITH ALL OTHER
FEDERAL, STATE AND LOCAL REGULATIONS. FERTILIZER APPLICATION SHALL
BE ACCOMPLISHED IN A MANNER THAT MINIMIZES THE RISK OF CONTACT
BETWEEN THE FERTILIZER AND SURFACE WATERS.
12. IF CONCRETE IS USED DURING CONSTRUCTION, THEN ALL NECESSARY
MEASURES SHALL BE TAKEN TO PREVENT DIRECT CONTACT BETWEEN
UNCURED OR CURING CONCRETE AND WATERS OF THE US. WATER THAT
INADVERTENTLY CONTACTS UNCURED CONCRETE SHALL NOT BE DISCHARGED
TO WATERS OF THE US.
13. ALL PROPOSED AND APPROVED TEMPORARY FILL AND CULVERTS SHALL BE
REMOVED AND THE IMPACTED AREA RETURNED TO NATURAL CONDITIONS
WITHIN 60 CALENDAR DAYS AFTER THE TEMPORARY IMPACT IS NO LONGER
NECESSARY. THE IMPACTED AREAS SHALL BE RETURNED TO ORIGINAL

CERTIFICATION.
5. ALL EROSION CONTROL DEVICES SHALL BE MAINTAINED ON ALL
CONSTRUCTION SITES, BORROW SITES, AND WASTE PILE (SPOIL) SITES,
INCLUDING CONTRACTOR OWNED OR LEASED BORROW PITS ASSOCIATED WITH
THE PROJECT. SUFFICIENT MATERIALS REQUIRED FOR STABILIZATION AND/OR

TREATMENT SHALL BE ON–SITE AT ALL TIMES.
6. SEDIMENT AND EROSION CONTROL MEASURES SHALL NOT BE PLACED IN
WETLANDS OR WATERS EXCEPT WITHIN THE FOOTPRINT OF TEMPORARY OR
PERMANENT IMPACTS AUTHORIZED UNDER THE PERMITS. EXCEPTIONS TO
THIS CONDITION REQUIRE WRITTEN APPROVAL FROM THE PROJECT ENGINEER.
7. EROSION CONTROL MATTING THAT INCORPORATES PLASTIC MESH AND/OR
PLASTIC TWINE SHALL NOT BE USED ALONG STREAMBANKS OR WITHIN
WETLANDS. EXCEPTIONS TO THIS CONDITION REQUIRE WRITTEN APPROVAL
FROM THE PROJECT ENGINEER.
8. AN NPDES CONSTRUCTION STORMWATER PERMIT (NCGO10000) IS REQUIRED
FOR CONSTRUCTION PROJECTS THAT DISTURB ONE OR MORE ACRES OF
LAND. THE NCGO10000 PERMIT ALLOWS STORMWATER TO BE DISCHARGED
DURING LAND DISTURBING CONSTRUCTION ACTIVITIES AS STIPULATED IN THE
CONDITIONS OF THE PERMIT. IF THE PROJECT IS COVERED BY THE PERMITS,
FULL COMPLIANCE WITH PERMIT CONDITIONS INCLUDING THE EROSION AND
SEDIMENTATION CONTROL PLAN, INSPECTIONS AND MAINTENANCE,
SELF–MONITORING, RECORD KEEPING AND REPORTING REQUIREMENTS IS
REQUIRED.
9. ALL WORK IN OR ADJACENT TO STREAMS SHALL BE CONDUCTED SO THAT
THE FLOWING STREAM DOES NOT COME IN CONTACT WITH THE DISTURBED
AREA. APPROVED BEST MANAGEMENT PRACTICES FROM THE MOST CURRENT
VERSION OF THE NC SEDIMENT AND EROSION CONTROL MANUAL, OR THE NC
15. ANY RIP–RAP REQUIRED FOR PROPER CULVERT PLACEMENT, STREAM
STABILIZATION, OR RESTORATION OF TEMPORARILY DISTURBED AREAS SHALL
BE RESTRICTED TO THE AREA DIRECTLY IMPACTED BY THE APPROVED
CONSTRUCTION ACTIVITY. ALL RIP–RAP SHALL BE PLACED SUCH THAT THE
ORIGINAL STREAM ELEVATION AND STREAM BANK CONTOURS ARE RESTORED
AND MAINTAINED, NOT RESULT IN THE DE–STABILIZATION OF STREAM BED
OR BANKS UPSTREAM OR DOWNSTREAM OF THE AREA, AND NOT RESTRICT
AQUATIC LIFE PASSAGE.
16. ANY RIP–RAP USED FOR STREAM OR SHORELINE STABILIZATION SHALL BE
OF SIZE AND DENSITY TO PREVENT MOVEMENT BY WAVE, CURRENT, OR
STREAM FLOWS, AND WILL CONSIST OF CLEAN ROCK OR MASONRY MATERIAL.
RIP–RAP SHALL NOT BE INSTALLED IN THE STREAMBED EXCEPT IN SPECIFIC
AREAS REQUIRED FOR VELOCITY CONTROL AND TO ENSURE STRUCTURAL
INTEGRITY OF BANK STABILIZATION MEASURES.
17. ALL MECHANIZED EQUIPMENT OPERATED NEAR SURFACE WATERS SHALL BE
INSPECTED, FUELED, LUBRICATED, AND MAINTAINED REGULARLY AND
MAINTENANCE PERFORMED IN A MANNER TO PREVENT CONTAMINATION OF
STREAM WATER FROM FUELS, LUBRICANTS, HYDRAULIC FLUIDS, OR OTHER
TOXIC MATERIALS.
18. HEAVY EQUIPMENT WORKING IN WETLANDS SHALL BE PLACED ON MATS OR
OTHER MEASURES TAKEN TO MINIMIZE SOIL DISTURBANCE.
19. THE CONTRACTOR OR OTHER RESPONSIBLE ENTITY SHALL REPORT ANY
PETROLEUM SPILL OF 25 GALLONS OR MORE; ANY SPILL REGARDLESS OF
AMOUNT THAT CAUSES A SHEEN ON SURFACE WATERS; ANY PETROLEUM
SPILL REGARDLESS OF SIZE THAT OCCURS WITHIN 100–FEET OF SURFACE
WATER; AND ANY PETROLEUM SPILL LESS THAN 25 GALLONS THAT CANNOT
BE CLEANED UP WITHIN 24 HOURS.
