This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as subsequently amended). Please be aware that Long Creek (segment index number 17-2-1-(1)) is an impaired water. This stream is impaired because both fish community and benthos have been rated as fair. There are also multiple Total Maximum Daily Loads (TMDLs) that affect this watershed for turbidity and fecal coliform. Information about these TMDLs can be found on Division of Water Resources' website: https://deq.negov/about/divisions/water-resources/planning/modeling-assessment/tmdls. In Part I, Section A, the General Permit explains that for discharges to watersheds affected by a TMDL, coverage under this permit may depend on the facility demonstrating that discharges have no reasonable potential to violate applicable water quality standards for specified pollutants. Please take notice that this certificate of coverage is not transferable except upon approval of the Division of Energy, Mineral, and Land Resources. The Division of Energy, Mineral, and Land Resources may require modification or revocation and reissuance of the certificate of coverage. This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Water Resources, the Coastal Area Management Act, or any other federal or local government permit that may be required. NCG140000 CERTIFICATE OF COVERAGE No. NCG140462 STORMWATER AND WASTEWATER DISCHARGES NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the Federal Water Pollution Control Act, as amended, Forsyth Redi-Mix, Inc. is hereby authorized to discharge stormwater and wastewater from a facility located at Redi Mix Concrete 309 S. Chimney Rock Road Greensboro Guilford County to receiving waters designated as Long Branch, class WS -IV waters in the Cape Fear River Basin, in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III, IV, V, and VI of General Permit No. NCG140000 as attached. This certificate of coverage shall become effective July 20, 2018. This Certificate of Coverage shall remain in effect for the duration of the General Permit.
