Dear Mr. Smith:
On June 14, 2018 Jason Robinson and Jeremiah Dow from the Raleigh Regional Office visited your
single-family residence (SFR)wastewater treatment system at 1316 Bahama Rd.,Bahama,NC to
evaluate compliance with the above Certificate of Coverage to discharge wastewater. The inspectors
spoke and left a package with Ms. Learner, who stated that Mr. Learner would call the inspectors to
discuss. As of the mailing of
.3 this letter, this office has not received a call from Mr. Learner.
The checked (15<l )boxes below show what conditions were observed at this system:
❑ In compliance:
❑ Your home is improperly plumbed: Some of the wastewater discharges are going directly
to the environment without first passing through the treatment system. This must be corrected
immediately.
n Disinfection: Your system is lacking disinfection, either chlorine tablets or a UV light
system. New rules put into place on August 1, 2007 require all SFR systems to have a means of
disinfection (and dechlorination when chlorine tablets are used to disinfect, if the system was
installed since that date). Since your system had no disinfection, the installation is to include a
chlorine tablet dispenser, a contact chamber capable of providing a minimum 30-minute contact
time, and another tablet dispenser that will hold dechlorination tablets.

E Treatment tablets missing or are wrong kind: You are responsible for always having
chlorine tablets and dechlorination tablets (if a required part of your system) in place. They
must be the kind for wastewater treatment and not for swimming pools. There did not appear to
be tablets in the chlorinator. No tablets were observed in the two tubes of the chlorinator.
Tablets should always be in the chlorinator, and should be contacting the flow of wastewater at
all times.

' ❑ Dechlorination: Your system was installed after August 1, 2007, so must have a means of
dechlorination located downstream of the chlorinator and its contact chamber. See Disinfection
paragraph above.
151 Pumping the septic tank: The septic tank should be pumped out every 3 to 5 years. A
pumping company can check the status periodically and determine when pumping is required.
_Ms.. Learner stated that the septic tank had been pumped in the last several years.
iz Failure to analyze the effluent: The effluent that is discharged from your system must be
analyzed once each year. See Part I(A) of your permit about his requirement. A list of NC
certified laboratories that provide this service was provided to you during the inspection.
Please have the effluent analyzed at a certified lab that was listed on the hand-out that was
provided, and submit a copy within 3 months of receiving this letter.
® Locations of treatment units: Please submit a map of the system. The map should show
the entire system, including the approximate location of where the sewerline exits the house, the
septic tank and sandfilter, the chlorinator and dechlorinator, and the discharge into the stream.
This map can be rough, and may be hand-drawn. Please submit this to this office within 30
days of receiving this.
NI Effluent pipe: The effluent pipe is where the wastewater drains from after it is treated in
the system. The inspectors were unable to find the effluent pipe, which according to early
inspections reports is at an iron pipe behind the wood pile to the southwest of the house. Please
make sure that the effluent pipe is discharging to the surface and the area around the discharge
is kept clear.
® Annual Fees/Permit Expired: The Division's database shows that the wastewater
discharge permit for this residence expired in 2012 and annual fees have not been paid since at •
least 2011 (see attached Notice of Violation letter from October 2016—NOV-2016-RV-0022),
a permit condition violation. Please contact Teresa Revis in the Division's Billing Department
