Dear Ms. Walsh:
The NC Division of Water Resources (DWR), Water Quality Regional Operations Section (WQROS),
Raleigh Regional Office(RRO),was notified on June 4,2018 of a fish kill in two ponds at one of your sites
permitted under WQ0003487 for Class A residuals distribution. This site is a 65-acre pasture field,located
east of the intersection of Shepard Road and Vicksboro Road in Norlina, Warren County,North Carolina.
You reported on June 6, 2018 that Novozymes personnel visited the site on 6/4/2018 and "dead fish were
observed in both ponds." You also reported that pond water samples were analyzed for dissolved oxygen
using a Hach handheld field probe.The results indicated that the dissolved oxygen ranged from 0.18 to 0.4
ppm.
The following item specifies a permit condition of WQ0003487 that was in violation as a result of the
run-off of residuals into the farm ponds that resulted in a fish kill:
III. Performance Standards, 1 which states: "The residuals management program shall be properly
maintained and operated at all times. The program shall be effectively maintained and operated as a non-
discharge system to prevent any contravention of surface water or groundwater standards. [15A NCAC
02T .1110]"
The information provided to date indicates that the residuals management program was not being properly
maintained and operated.
You are required to take any necessary action to correct the above violation and to provide a written
response to this Notice within 30 days of receipt of this correspondence. Please include in your response
