The setbacks listed on Sheet N-1.3 are for 15A NCAC 18A .1950 as opposed to 15A NCAC 02T .0506. The setbacks appear to meet the 2T requirements. 8. The wet weather storage pond does not have a liner, however, the engineer states that depth to bedrock >4ft. 2. Recommended application rate of 0.25 in/hr – see comment B. page 2 from Patrick Mitchell. 3. Attachment B – 6 Zones – see comment C. page 2 from Patrick Mitchell. DocuSign Envelope ID: A4BE56AB-C079-45FB-AFF8-E291D0B8828C 6/15/2018 Here are my comments for the Golden Valley RV Park application:  Site map – A compliance/review boundary map is provided on page N-1.3 of the conceptual design drawings. Due to the irregular waste boundary for the Golden Valley RC Park drip irrigation field, portions of the proposed compliance boundary exceed the 250-foot setback. The attached map highlights the sections that need to be redrawn. You will be required to submit an updated site map for approval.  Monitoring wells – The permittee will be required to construct three down gradient monitoring wells and one background monitoring well. Two of the down gradient monitoring wells will be dedicated to the west side of irrigation field. One down gradient monitoring well will be dedicated to the east side of the irrigation field. The background well must be located outside the influence of the irrigation field. It is recommended that the down gradient monitoring wells be located where the review boundary intersects gullies 1, 2, and 5 as depicted on Figure 7 of the Hydrogeological Assessment Report. Prior to well construction, the permittee will be required to submit a work proposal to the Asheville Regional Office for approval.  Groundwater monitoring analytes – Groundwater monitoring will be conducted 3 times per year in the months of March, July, and November. Water samples will be analyzed for Total Organic Carbon (TOC), Chloride, Fecal Coliform, Total Ammonia, Total Nitrate, pH, Total Phosphorus, Total Dissolved Solids (TDS), Volatile Organic Compounds (VOC), and water level. VOC analysis is only required in the month of November. Prior to sampling, the permittee will also be required to monitor for temperature, specific conductivity, dissolved oxygen, and oxidation reduction potential at each monitoring well.
