As part of the Pretreatment Compliance Inspection (PCI) conducted on March 20, 2018 the
requirements or the Pretreatment Annual Report (PAR) were discussed.
Attached are copies of the Pretreatment Performance Summary (PPS), the Significant Non -Compliance
Report (SNCR) and the Program Info sheet.
Pretreatment Performance Summary (PPS): Review this form and ensure that the numbers entered are
correct. In particular, confirm that a Notice of Violation (NOV) (question #16) was sent to Carolina Dairy
in 2017 for significant noncompliance (SNC) violations that occurred in 2016.
Significant Non -Compliance Report (SNCR): The form indicates two significant industrial users (SIUs)
were in significant noncompliance (SNC) for both 6-month reporting periods: Carolina Dairy for BOD
violations and Grede II for failure to self -monitor. Confirm that this information is accurate.
Narrative: Prepare a narrative summary discussing SIUs in SNC. Note the reasons and which 6-month
period they were in SNC. If SNC for limits, note if it was due to chronic, technical review criteria (TRC) or
both. Explain how a SNC situation was or will be resolved (e.g. increased limit, improved pretreatment,
production changes, etc.)
Public Notice: Publish in the local paper a list of SIUs in SNC. An example notice was provided to you
during the PCI.
Program Info sheet: Review the Program Infor sheet. Make any necessary corrections to the contact
information. Take note of the due dates.
Compliance Judgment worksheet: Review the attached compliance judgment worksheets and confirm
that all the available data is included. The calculations indicate that Carolina Dairy is SNC for BOD for
both 6-month reporting periods. Carolina Dairy was also in SNC for BOD in the second half of 2016. The
Town of Biscoe must act to bring the SIU back into compliance. Examples of appropriate action may be
to modify the IUP limit or place the SIU under a Consent Order.

Submit the completed PAR to PERCS no later than April 30, 2018. Include the PPS form, the SNCR form,
the narrative, affidavit of Public Notice or copy of the notice from the newspaper, the Program Info
sheet with any corrections and copies of the NOVs issued to Carolina Dairy and Grede II for SNC
violations.

Notice. Listed below are Significant Industrial Users that were in significant noncompliance
(SNC) with national pretreatment regulations, 40 CFR Part 403, and state pretreatment
regulations, 15 NCAC 2H .0900, and local pretreatment regulations:
AAM Casting— Failure to monitor; 3rd quarter 2017
Carolina Dairy— BOD violations; all 4 quarters of 2017
A continuing effort is being made by all the listed industries to achieve compliance, including
installation of new equipment and upgrading of existing equipment and continued progress is
expected until full compliance can be attained.

Carolina Dairy
On 3-12-18 The Town Board of Commissioners voted to drop the daily
average for BOD and TSS.
And change the monthly avg. for BOD and TSS
BOD was 250 mg/L changed to325mg/L
TSS was 250 mg/L changed to 300mg/l
AAM CASTING
After a meeting with AAM Casting monitoring reports were received,
for quarters 1,2,4. and was in compliance. A Violation for quarter 3
was issued and mailed. The QA Plant Engineer is now working with
AAM Staff and the Health and Safety Engineer to make sure samples
are taken on time and all logs a up to date and in order.

Requirements:
1. Perform on -site inspection to determine compliance status of Grede and Flakeboard.
2. Issue NOV's to Carolina Dairy, Grede/American Axle, and Flakeboard America per the Enforcement Response Plan (ERP) and
issue a public notice of significant non-compliance in the local newspaper.
3. Grede/American Axle: It is unclear if any process wastewater is discharged to the sanitary sewer. Confirm how process
wastewater is disposed of and take appropriate action — issue a permit containing the categorical requirements or rescind permit as not
necessary. Grede is responsible for quarterly sampling if IUP is not dropped prior to March 31, 2018.
4. Flakeboard/Aruaco: Rescind the permit before May 31, 2018 or commence sampling by June 30, 2018.
5. Modify permit to revise daily limit on BOD for Carolina Dairy or place the SIU under a Consent Order
6. Implement monitoring plan beginning in the 2" quarter of 2018 and collect 4 quarterly samples ending with the V quarter of 2019.

Pretreatment Inspection Response
1. Inspections on Grede/AAM Castings. Was preformed on 5-2-18
Flakeboard /Arauco was inspected on 3-20-18
2. On 5-21-18 NOV'S was issued to Carolina Dairy and AAM Casting
and a public notice was sent to the local News paper
please see email's for NOV'S / SNC
3. AAM Castings was inspected on 5-2-18 and everything was
in order per. The permit. The town is waiting on the paper work
for the first quarter.of 2018. They have been sent an email reminding
them of quarterly samples.
4. Flakeboard/Arauco asked the Town of Biscoe to rescind there
permit on 5-8-18. They are doing a pump and haul, with a
contractor. Only domestic sewer is being discharged.
5. On 3-12-18 the Town Board voted to revise the permit for
Carolina Dairy. They dropped the daily average on BOD and TSS.
The TSS monthly avg. went from 250 mg/l to 300 mg/1
The BOD monthly avg. Went from 250 mg/1 to 325 mg/1
6. Arrangements have been made with Meritech,Inc. To start sampling
for the monitoring plan starting in the 2 quarter of 2018 and ending
in the first quarter of 2019.
