Dear Mr. Shaw:
On May 23, 2018 staff of the North Carolina Division of Water Resources (DWR), Winston Salem
Regional Office performed a compliance inspection of the subject facility and the residuals surface
disposal program. This inspection was performed by DWR staff person Justin Henderson. The
compliance inspection report is enclosed for your review. While the inspection mostly reflects
compliance with permit No. WQ0010528, the following items of concern require your attention:
1) Expired pH buffers were being utilized for pH meter calibration. To clearly
. demonstrate compliance with pathogen and vector attraction reduction analysis, pH meter
pre-analysis calibration must be performed with valid pH buffers.
2) Permit Condition V. (2.) states that prior to each bulk residuals surface disposal event, the
Permittee or his designee shall inspect the residuals storage,transport, and application
facilities to prevent malfunctions, facility deterioration and operator errors resulting in
discharges. The Permittee shall maintain an inspection log that includes, at a minimum, the
date and time of inspection, observations made, and any maintenance, repairs, or corrective
actions taken. These inspection log shall be maintained for a period of five years from the
dated of inspection, and made available to the Division upon request. The above described
inspection log was not available for review during this inspection.
This is a repeat concern that was previously brought to your attention in a compliance
letter dated May 5, 2017.

In addition to the above described items of concern, please note the following reminder:
a) Permit Condition III. (5) requires that the spill prevention and control plan be maintained in
all residuals transport and disposal vehicles.
This office requests that hauling records for the water treatment plant residuals disposal event
performed on May 22, 2018 be submitted to this office within 30 days following receipt of this
correspondence. Our office appreciates your diligence in addressing the above described items of
