Mr. Madix,

Thanks for talking with me a few weeks ago and for sending a copy of the renewal application
for your facility. As I indicated in the call, I will be working on this renewal process. I have a
few other permits I am drafting right now, but will probably be starting with your facility
within the next week. Since the previous renewal application came in back in 2013 I'm
guessing there may be some changes that we should discuss and possibly have documented
for the renewal. In addition, here are a few additional items that would be helpful if you can
provide updates:
• I have attached a printout of contacts we have in our database for the facility. Can you
please review this and let me know the changes to contacts that we need to make and
if the owner/facility information is still accurate?
• We will have to work to get accurate outfall location information in our system in order
to meet federal requirements for electronic reporting. If you could provide the lat/long
coordinates for your outfall points that would be helpful.
• If you can provide summary monitoring information since your application submittal in
October 2013 that would be help as well.
• Let me know about any changes that have occurred in your facility or processes since
your application submittal. If there have been changes that have led to site plan
updates, please provide a more recent copy of the site plan.
Once I get started on drafting the permit it may be useful for us to have conference call to talk
about your site and the renewal process.

First of all I wanted to apologize for the time it has taken us to get to the renewal request
for this facility. We realize that you have been waiting a while for this process to start
and appreciate your continued implementation under the previous permit. I wanted to
make sure that you are still our contact person for the stormwater permit for this facility.
If not, can you please direct me to the correct contact.
I would like to set up a conference call to discuss the renewal process when that works
for appropriate staff with DART Acquisitions LLC. I am a part time employee so I'm only
in the office part of the week. I am out of the office the rest of this week but should be
back in Monday — Wednesday of next week (April 16-18). If there are time early next
