Dear Mayor Shaw:
The Division received your request to reduce the design flow of the subject facility to 0.045 MGD. We do not
object to your request. We herewith forward this modified COC pursuant to the requirements of North Carolina
General Statute 143-215.1,the Memorandum of Agreement between North Carolina and the US Environmental
Protection Agency(EPA)dated October 15,2007(or as subsequently amended).
This modification includes the following changes:
• A revised Supplement to CoC page,showing the new design flow
• Flow monitoring changes from Continuous/Recorder to 2/Month—Instantaneous,effective May 1,2018.
If any parts,measurement frequencies or sampling requirements contained in the General Permit are unacceptable
to you,you have the right to request an individual permit by submitting an individual permit application.Unless
such demand is made,the certificate of coverage shall be final and binding.
This CoC is not transferable except after notice to the Division.The Division may modify,or revoke and reissue
this CoC.This CoC does not affect your legal obligation to obtain other permits that may be required by any other
Federal,state,or local government regulation.

SUPPLEMENT to CoC
In accordance with the NCG590000 General Permit and the information provided in the permittee's application,
the Division determined Effluent Limitations and Monitoring Requirements under General Permit NCG590000,as
presented below.The Permittee is required to notify the Division of any modifications made to the water
purification process,the wastewater treatment processes,and/or revised usage of chemical(s)at the plant.In case
of such changes,the Permittee shall apply to modify this COC, if necessary.This notification shall be made in
writing to the DWR/NPDES Permitting Unit.
This discharge is identified as:
1. Type of Water Purification System identified: Greensand X Conventional
2. Design Flow(MGD): 0.045 MGD filter backwash wastewater[1.5 MGD potable water production]
3. Receiving Stream Classification: C
4. Effluent Limitations and Monitoring Requirements is subject to in this General Permit as:
GREENSAND WTP discharging to FRESHWATER[See Part 1,Section B. (1.)]
GREENSAND WTP discharging to SALTWATER[See Part 1,Section B.(2.)]
X CONVENTIONAL WTP discharging to FRESHWATER[See Part 1,Section B.(3.)]
CONVENTIONAL WTP discharging to SALTWATER[See Part 1,Section B.(4.)]
Beginning on the effective date of this COC,all Discharge Monitoring Reports(DMRs)shall be submitted
electronically using NC DWR's electronic discharge monitoring system(eDMR)system.
