This letter is to reyuest formaljurisdictional area delineation verification for the US 64 widening trom
Lake Toxaway to Indian Creek ('I'IP R-2409 C) in Transylvania Coimty. Enclosed is a topographic
map depicting all streams delineated wi[hin projec[ area, as well as completed NCDWQ Stream
IdentiCcations Irorms and Jurisdictional Determination Forms (Kapanos Forms) for each stream. There
are sevenjurisdic[ional streams in the project area, six of which have proposed crossings. Attached is
a table indicating jurisdictional status and proposed impacts for cach alternative.

roadside pullover located on the north side of US 64 immediately cast of thc Lake Toxaway outfall, as
depicted on [he enclosed map.
