Chris,
On March 29, 2018, the Division of Water Resources (Division) received your application dated March 29, 2018,
requesting a 401 Water Quality Certification from the Division for the subject project. The Division has determined that
your application is incomplete and cannot be processed. The application is on hold until the following items are
addressed:
The stream impact table does not match the project description in the PCN nor Sheet R1-02. Specifically, impact
3 is listed at 22 feet on the table and 42 feet in the project description and map. Please revise and resubmit the
stream impact table or explain the discrepancy. [15A NCAC 02H .0502(d)]
2. The application is not signed by the owner/applicant and no agent authorization was provided. Please confirm
that the agent authorization provided with version 1 of the subject project may be incorporated into version 2,
provide an agent authorization form, or provide a copy of the PCN signed by the owner/applicant. [15A NCAC
02H .0502(f)]
Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the
proper consideration of the application. If all of the requested information is not received within 30 days, the Division
will be unable to approve the application and it will be returned.
